Percutaneous Transhepatic Cholangiography and the Role of Interventional Radiology in Biliary Obstructions

Abstract

The interventional radiologist has an important role in the decompression of the obstructed biliary tree. In general, their skills are used for patients who cannot tolerate anesthesia due to frailty or hemodynamic collapse or in those where ERCP was unsuccessful due to the nature of the obstruction. A percutaneous hepaticogastrostomy can be employed to decompress the biliary tree. The technique of percutaneous transhepatic cholangiography can also clear common bile duct stones and biopsy masses that may be causing the obstruction, define the biliary tree anatomy, and provide temporary drainage of the biliary tree.
