Résumé
Pour un psychiatre clinicien, l’étude de la douleur dans la schizophrénie se présente comme un paradoxe : les plaintes douloureuses sont relativement peu exprimées par les patients atteints de schizophrénie, contrairement à d’autres cadres psychiatriques comme la dépression ou les troubles anxieux ; de plus, lorsqu’une plainte somatique est formulée, elle peut renvoyer aux formes cénesthésiques ou hypocondriaques de la maladie. La douleur peut donc s’avérer doublement déroutante pour le clinicien par son absence (hypoalgésie) mais également par son excès, sa « bizarrerie » ou son caractère médicalement trompeur. La plainte douloureuse des patients doit donc être recherchée, parmi d’autres indices des multiples maladies physiques qui les menacent, mais elle doit également être remise, le cas échéant, dans le contexte de la maladie psychotique elle-même.
