Human Herpesviruses: Varicella and Herpes Zoster

Abstract

Varicella-zoster virus (VZV) causes two clinically distinct diseases in humans. Varicella (or chickenpox), resulting from primary infection of a susceptible individual, is a common, extremely contagious, and usually benign acute illness characterized by a generalized vesicular rash. In unvaccinated populations, varicella occurs almost exclusively in children in seasonal epidemics. Like all other herpesviruses, VZV establishes latency following primary infection. Reactivation of latent VZV from sensory ganglia results in herpes zoster (or shingles), a painful localized cutaneous eruption in a dermatomal distribution that is most common among elderly individuals. Thus, varicella results from primary exogenous exposure to VZV, while herpes zoster represents endogenous reactivation of latent VZV that usually occurs decades after the primary infection. The incidence of herpes zoster is about 3–5 cases per 1000 person-years, with a clear association between aging and higher incidence. Overall, the incidence of herpes zoster appears to be increasing. Varicella and herpes zoster can cause significant disease in immunocompetent patients and life-threatening morbidity in immunocompromised patients, but antiviral treatment is available. Varicella and herpes zoster vaccines are highly effective at preventing VZV disease and attendant complications in both immunocompetent and immunocompromised persons. Requirements for universal childhood varicella vaccination in the USA have reduced the incidence of chickenpox (and its complications, including death) by over 95%. More recently, a nonreplicating vaccine for prevention of herpes zoster has been introduced. The recombinant herpes zoster vaccine was about 90% effective in clinical trials, although to date only 30–35% of adults in the USA over the age of 60 have been vaccinated.
