Abstract

Schizophrenia is a chronic, debilitating illness that afflicts approximately 1% of the adult population. Before the discovery of antipsychotic medications, effective treatments for schizophrenia were not known, and most patients were destined to spend their lives languishing in state hospitals with little hope of returning to the community. Between 1950 and 1965, the widespread use of antipsychotic medications, coupled with the spiraling costs of long-term inpatient treatment for mental illness, led to the “deinstitutionalization” movement, the locus of most psychiatric treatment shifting from the hospital to the community (Johnson, 1990). The change in treatment locus is reflected in the reduction of hospital beds in the United States occupied by patients with psychiatric illness, from 560,000 in 1955 to under 140,000 in the mid-1970s (Meyer, 1976) and the drop in average length of hospital stay from six months to three weeks (Sharfstein, 1984).
