Abstract
Tubular injury in the form of acute interstitial nephritis (AIN) or acute renal failure is a well-recognized complication of the administration of a variety of drugs or diagnostic agents, such as radiocontrast dye (Linton et al., 1980). The acute onset of AIN as well as the frequently associated findings of fever, rash, eosinophilia, and elevated IgE levels have strongly suggested an allergic or hypersensitivity reaction. The underlying immunologic mechanism(s) of these reactions has not been established and it will be the purpose of this chapter to examine the possible role of immediate hypersensitivity.
