Transbronchoscopic Emphysema Treatment with One-Way Zephyr Valves

Abstract

Few alternatives are available for emphysema treatment. The high mortality associated with volume reduction surgery (LVRS) and lung transplantation has greatly restricted their applicability. As a result, minimally invasive alternatives have been developed using the same rationale underlying LVRS—to decrease or prevent air trapping in lung that is damaged by emphysema with reexpansion of the remaining tissue. This chapter will focus on one of these minimally invasive approaches, namely, transbronchoscopic placement of one-way valves. This technique is safe and reversible, although it is still limited by the absence of a clear understanding regarding the best selection criteria. This gap is due in part to a poorly designed trial, which ended up slowing down the acceptance of this promising technique.
