Abstract
Transmission of the human immunodeficiency virus (HIV) through transplantation of organs and tissues has been reported infrequently, in most cases from donors whose sera had not been tested for HIV antibody.’ Screening of potential organ and tissue donors for HIV infection has greatly reduced the risk of HIV transmission through transplantation: Only 11 transplant recipients have been reported to have been HIV-infected through transplantation since the availability of donor serologic screening in 1985. Of these recipients, one was transplanted urgently with a liver before the result of the donor’s positive antibody test was available2; one received a kidney from a living donor who seroconverted after having been screened 8 months before organ donation3; two received organs from a donor whose serum tested falsely negative after hemodilution from transfusion of a large volume of blood4-5; and seven received organs or tissues from a seronegative donor who was thought to have very early HIV infection.6
