Abstract
The repair of tetralogy of Fallot has undergone many changes and revisions over the years, leading to the current use of a transatrial, transpulmonary artery approach for most cases. Transannular incisions, when needed, are short, mainly because the ventricular incision is not needed for closure of the ventricular septal defect (VSD), as the VSD is generally closed via the right atrium. The ventricular incision therefore need not be any larger than necessary to establish an adequate right ventricular outflow tract (RVOT). Several simple technical maneuvers will allow excellent exposure to accomplish this repair via the transatrial, transpulmonary artery approach.
