Begleittherapie bei Cisplatin

Zusammenfassung

Eine Cisplatintherapie sollte nur durchgeführt werden, wenn folgende Punkte Beachtung finden:

  • Serumkreatinin im Normbereich, Kreatininclearance > 60 ml/min,

  • Normwerte für Elektrolyte im Serum (Kalzium, Magnesium),

  • keine manifeste Harnabflußbehinderung (gegebenenfalls Pigtail- oder Nierenfistelanlage),

  • keine Begleiterkrankungen, die Hydratation mit 2–3 l/m2/Tag verhindern,

  • ausreichende Urinproduktion (100–200 ml/h),

  • Meiden zusätzlicher nephrotoxischer Substanzen (z.B. Aminoglykosidantibiotika),

  • Bestrahlung im Nierenbereich erhöht das Nephrotoxizitätsrisiko,

  • keine schwerwiegenden Vorschädigungen des Innenohrs (Audiogramm obligat) und des peripheren Nervensystems.
