Résumé
Depuis leur introduction dans les services de cancérologie, les psychologues et les psychiatres ont été amenés à redéfinir des modalités d’interventions psychothérapeutiques adaptées aux patients atteints de cancer. Cet article propose une réflexion générale sur la définition d’une « psychothérapie » et celle du « psychothérapeute » qui nécessite une disposition psychique et une formation spécifique différentes de celles visant l’évaluation psychopathologique diagnostique. En oncologie, outre la prise en compte de la réalité somatique, le cadre psychothérapeutique requiert de s’ajuster à la demande spécifique de ces patients souvent en souffrance psychique mais non toujours demandeurs d’une « psychothérapie », d’où des réponses thérapeutiques variées. Au-delà de l’application stricte d’un cadre théorique, toute pratique de la psychothérapie impliquerait de la part du psychothérapeute un questionnement sur son propre fonctionnement psychique et sur la spécificité du contre-transfert en oncologie.
Abstract
Since they were introduced in oncology departments, the psychologists and the psychiatrists were brought to define new modalities of psychotherapeutic interventions adapted to cancer patients. This article suggests a general reflection on the definitions of a « psychotherapy « and of the « psychotherapist » which needs a psychologic way of being and a specific training different from the ones aiming the psychopathological diagnostic evaluation. In oncology, in addition to the importance of taking into account the somatic reality, the psychotherapeutic setting also requires to adjust to the suffering cancer patients’ specific demand who are not always feeling a need for a psychotherapy in the strict sense. So, different therapeutic responses have to be proposed. Beyond a strict application of the theoretical framework, every practice of the psychotherapy should involve a questioning from the psychotherapist upon his proper mental functioning as well as specific contertransference in oncology.
