The etiology of ONFH is multifactorial, but alcohol abuse [5], high-dose corticosteroid use [6], and autoimmune diseases [7] are the 3 most commonly accepted associated risk factors. Up to now, however, there are a substantial number of patients who have no identifiable risk factors, and they were diagnosed with idiopathic ONFH [1,20].

The epidemiological survey revealed that adjusted odds ratio of the patients with ≧3000 drink-years (weekly ethanol consumption × years) was 3.86 compared with never-drinkers [11]. ONFH due to heavy alcohol intake is likely caused by lipid metabolism disorders and increased blood cortisol [12,13].
