Ergebnisse operativ behandelter lateraler Klavikulafrakturen

Zusammenfassung

Die vorliegende Studie befaßt sich mit den Operationsergebnissen von 41 Patienten mit lateraler Klavikulafraktur, die an der Staatlichen Orthopädischen Klinik Harlaching der LMU München vom 31.10.79 bis zum 31.10.94 behandelt wurden. Es konnte festgestellt und bestätigt werden, daß die lateralen Klavikulafrakturen durch ihre Vielgestaltigkeit eine Sonderstellung unter den Klavikulafrakturen einnehmen und ein differenziertes therapeutisches Vorgehen erfordern. Unter den Operationsmethoden war die Kirschner-Drahtzuggurtungsosteosynthese mit 24 Fällen die mit Abstand am häufigsten gewählte; 2mal wurde die einfache Spickung mit Kirschner-Draht ohne Cerclage verwendet. Weitere 12 Frakturen wurden mit Plattenosteosynthesen versorgt; 3 Frakturen mußten durch eine Resektion therapiert werden, da sich unter konservativer Therapie 2mal eine Pseudarthrose und eine starke AC-Gelenksarthrose gebildet hatte. Das subjektive Ergebnis der 41 nachuntersuchten Patienten zeigte in 92,7% der Fälle gute und sehr gute Ergebnisse. Die klinischen Ergebnisse von 36, im Durchschnitt nach acht Jahren nachuntersuchten und 33 radiologisch beurteilten Patienten ergaben in 97% der Fälle gute und sehr gute Werte im Boenisch-Score.

Abstract

This retrospective study presents the results of 41 patients with a lateral clavicle fracture eight years after the treatment at the Staatliche Orthopädische Klinik of the LMU München in the time between 31.10.1979 and 31.10.1994. It could be shown that the lateral clavicle fractures, due to their variety, hold a special position among the clavicle fractures and thus need a differanciated therapeutic procedure. Among the operation methods the K-wire stabilisaton with cerclage was the most often chosen operation technique i.e. in 24 cases. The K-wire without cerclage was used in two cases. Twelfe fractures were treated with AO-plate-osteosynthesis. Three fractures had to be treated by resection, as they produced two pseudarthrosis and one serious AC-arthrosis during the conservative therapy. 92,7% of the subjective results of the 41 checked up patients were good and very good. The clinical follow-up of 36 and the radiological follow-up of 33 patients revealed in 97% a good and very good outcome in the Boenisch-Score.
