Zusammenfassung

Hintergrund:

Klinische Studien stellen einen zentralen Bestandteil klinischer Forschung dar. Die Rekrutierung von Patienten wird durch unterschiedlichste Variablen auf Patientenseite, durch das Studienprotokoll sowie Motivation und Verhalten des Prüfarztes bestimmt.

Hauptthesen:

Die Motivation zur Teilnahme an einer klinischen Studie wird auf Patientenseite von der Grunderkrankung, dem möglichen Nutzen der Studienteilnahme, dem Aufwand, dem Studienprotokoll und dem Vertrauensverhältnis zum Arzt bestimmt. Randomisierte Studien, ein hoher Aufwand für den Patienten und ein Plazeboarm in einer kontrollierten Studie stellen Barrieren für die Teilnahme dar. Kenntnisse zum Kommunikationsverhalten und zu den Bedürfnissen von Patienten sind notwendig, um in einem gut geführten Aufklärungsgespräch die Grundlage für eine eigenverantwortliche Zustimmung des Patienten zur Studienteilnahme und eine dauerhafte Compliance zu erreichen. Die ärztliche Motivation, als Studienarzt tätig zu werden, wird u. a. von den Gegebenheiten im Krankenhaus oder in der Praxis, dem Aufwand und dem wissenschaftlichen Eigeninteresse bestimmt.

Schlussfolgerung:

Die Kenntnis positiver und negativer Einflussfaktoren auf die Bereitschaft zur Teilnahme an einer klinischen Studie kann helfen, Strategien zur erfolgreichen Rekrutierung von Patienten zu entwickeln.

Abstract

Background:

Clinical trials are essential in clinically orientated medical research. Enrollment of patients is determined by patient-related barriers, protocol-related barriers, and motivation and behavior of the physician.

Main Theses:

Patients enrollment in clinical trials is determined by disease, possible diagnostic or therapeutic benefit of study procedures, effort of protocol procedures, and trustfulness of the relationship between the physician and patient. Randomized trials, extensive study procedures and the possibility of receiving placebo are barriers to participation. Understanding of the communication process and knowledge of patients' needs are the base education to gain patients' informed consent to enter a clinical trial and a stable compliance in the course of the study. Motivation of physicians to enroll patients in a clinical trial is determined, e. g., by environmental conditions in clinic or doctors' office, the time and effort of study procedures, and individual academic interest.

Conclusion:

Awareness of factors (positive and negative) which influence motivation of participation in a clinical trial is useful to provide strategies for successful enrollment of patients.
