Abstract
Van Bergeijk H, Berger MY. Treatment of urinary tract infections inside and outside of surgery hours. Huisarts Wet 2008;51(9):430-4.
Introduction Urinary tract infections (UTIs) are responsible for 1,19% of all contacts with general practitioners (GPs) in Holland, taking 8th place in all medical contacts presented to a GP. In the Netherlands, there are special Emergency GP Practices that provide medical care given by GPs outside office hours. The first aim of this research project is to describe the adherence to the medical treatment guidelines for UTIs by Dutch GPs. Secondly, we wanted to compare the adherence to these guidelines within GPs own regular practices to the adherence at the out of office hours service.
Method Retrospective study at 1 out of office hours service and 24 regular general practices.
Results In total 470 UTIs were included. In 50,4% of the cases there was adherence to the guidelines for medical treatment. At the out of office hours service, 52,3% (34/65) of the cases were complicated infections, versus 25,9% (105/405, P<0,001) at the regular practices. Of the complicated infections at the out of office hours service, 32,4% (11/34) were treated according the guidelines versus 35,2% (37/105, P = 0,758) at the regular practices. With uncomplicated infections this was 45,2% (14/31) versus 58,3% (175/300, P = 0,158).
Conclusion There are more complicated UTIs presented at the out of office hours service than at a regular practice. Complicated UTIs are less frequently treated according medical guidelines than uncomplicated UTIs. There is no difference in adherence to guidelines between an out of office hours service and regular general practice.
