임상실습을 경험한 간호대학생의 다제내성균 감염관리 인식과 감염관리역량

Abstract

Purpose: The aim of this study was to identify awareness and competency for Multi-Drug Resistant Organisms (MDRO) infection control in nursing students with experience of clinical practice. Methods: This cross-sectional descriptive study was conducted from March 2019 to May 2019 by including 231 nursing students in four nursing schools located in Seoul, Gyeonggi-do and Chungcheongnam-do. The data were collected using self-report questionnaires. Results: The awareness and the competency for Carbapenem-Resistant Enterobacteriaceae (CRE) infection control were lower than that of Methicillin-Resistant Staphylococcus aureus (MRSA). The agreement between the awareness and the competency of MDRO infection control in participants was low with regard to isolation, contact precautions, and disinfection for MRSA. Also, it was low with respect to disinfection, isolation, contact precautions, and carrier identification for CRE. The awareness and the competency of MDRO infection control exhibited significant positive correlation. Conclusion: The infection control competency is required to prevent MDRO infection. In order to enhance the infection control competency, it is important to raise awareness about MDRO infection control by providing education based on the guidelines and the principles of infection control.

서 론

1. 연구의 필요성

1941년 이래로 질병을 치료하거나 치료과정 중에서 발생될 수 있 는 감염을 예방하기 위하여 많은 항생제가 사용되고 있다[1]. 전신 성 항감염약과 항균물질 소비량은 2008년에 32.8과 26.9 (단위: DDD/1,000인/일)에서 2016년에 44.3과 34.8로 증가되었고, 전년 대 비 11.0%와 10.5%가 상승되었다[2]. 이와 같이 항생제의 과도한 사용 은 해당 약제 뿐 아니라 타 약제에도 영향을 주는 ‘내성’을 유발한다 [3]. 하나 혹은 그 이상의 항생제에 내성을 보이는 다제내성균은 항 생제 사용을 비효과적으로 만들고, 중증도 감염 환자의 치료에 심각한 영향을 주게 된다[4]. 즉, 다제내성균에 감염된 환자는 항생제 사용의 범위 제한, 재원기간의 증가, 질병치료 지연 및 사망률 증가와 같은 부작용을 경험한다. 질병관리본부(Korea Centers for Disease Control and Prevention, KCDC)는 2010년 12월에 다제내성균과 관련한 감염병의 예방 및 관리에 대한 법률을 전면 개정하였고, 그 종류를 반코마이신 내성 황색포도알균(Vancomycin-Resistant Staphylococcus aureus, VRSA), 반코마이신 내성 장알균(Vancomycin-Resistant Enterococcus, VRE), 메티실린 내성 황색포도알균(Methicillin-Resistant Staphylococcus aureus, MRSA), 카바페넴 내성 장내세균속균(Carbapenem-Resistant Enterobacteriaceae, CRE), 다제내성 녹농균(Multidrug-Resistant Pseudomonas aeruginosa, MRPA) 및 다제내성 아시네토박터 바우마니균(Multidrug-Resistant Acinetobacter baumanii, MRAB)으로 규정하였다. 이 중 VRSA 감염과 CRE 감염은 2017년 6월에 제3군 감염병으로 지정되어 전수 감시 및 지체 없이 신고하도록 시행되었다[5].
황색포도알균(S. aureus)은 건강관련 감염의 흔한 원인 중 하나이고, 중대한 합병증과 사망률을 야기한다[6]. 황색포도알균주는 페니실린에 내성인 동시에 페니실린 분해효소에 안정된 페니실린 계통인 옥사실린과 메티실린에도 부정적 영향을 받으며, 세파로스포린, 카르바페넴, 암피실린 및 피페라시린에도 내성이 있다[7]. 우리나라에서 황색포도알균의 내성률은 60.0~75.1%로 높은 내성률을 나타내고 있고, 요양병원에서는 약 80.0% 이상이라고 보고하였으며, 주요 분리원은 호흡기, 농양, 상처 순으로 나타났다[8]. MRSA는 정상적인 인체에서도 질병을 유발할 수 있는 많은 독성을 갖고 있고, 치료 범위의 제한과 접촉 감염이 가능하기에 의료기관에서도 중요하게 다루는 감염 중 하나이다. CRE 중 카바페넴 분해효소 생성 장내세균속균(Carbapenemase-producing Enterobacteriaceae, CPE)은 카바페넴 분해 유전자를 가진 CRE를 뜻하는데, 다른 균주에 내성을 전달할 수 있는 가능성이 있어서 더 위험하다[9]. 즉, 박테리아 DNA(Deoxyribonucleic acid) 주변의 플라스미드가 쉽게 분리되고, 다른 인체로 빠르게 침투되어 집단 감염을 일으킬 수 있다. 국내에서는 2010년 이후로 다양한 CPE가 확인되고 있고, 그 숫자도 매년 크게 증가하는 추세이다[8].
최근 병원 외에 지역사회에서도 다제내성균 감염(Community Acquired MRSA, CA-MRSA)이 점점 증가되고, 병원 획득성 MRSA와 지역사회 획득성 MRSA가 병원과 지역사회에 공동으로 침투되고 있다[10,11]. 이는 의료기관 내 균과 지역사회 내 균이 서로 전파되고 있다는 현상으로 보인다. 이러한 전파는 보균자, 환자, 의료인, 주변 환경 접촉자가 감염원으로 작용할 수 있으므로 다제내성균과 관련된 감염관리는 이제 더 이상 병원 내 직원만을 대상으로 국한해서는 안되고, 환자를 대상으로 하는 모든 이가 내성균 집락화 및 전파를 감소시키기 위한 노력을 계속해야 된다. 특히 침상 옆에서 환자를 직접 간호하는 간호사의 경우 감염관리의 핵심적인 위치에 있기에 더욱 그러하다. 따라서 간호사를 준비하는 간호대학생도 다제내성균 감염에 관한 심각성과 중요성을 인식해야 한다. 임상실습에 참여하게 되는 3-4학년 간호대학생은 임상실습기간 동안 병원 내에서 간호사의 감독 하에 환자의 활력징후 측정, 혈당 검사 및 이송보조 등을 수행하며 환자 주변 환경을 자주 접촉하기에[12] 이들을 위한 연구가 필요하다. 하지만 학교에서는 이와 관련된 교과목이나 프로그램이 드물고, 다제내성균에 관한 지식도 미생물학과 약리학에서 일부 다루고 있다[13]. 감염관리 중 격리에 대한 사항은 기본간호학에서 기초적으로 다루고 있지만 다소 부족하다. 한국간호교육평가원에서 제시한 핵심기본간호술 20가지 중 하나의 항목으로 ‘보호장구 착용 및 폐기물관리’라는 주제가 있으나, 다제내성균 감염관리에 대한 주된 내용이 아닌 보호장구 착용방법과 사용한 보호장구 폐기절차에만 머무르고 있는 실정이다.
다제내성균 감염관리에 대한 인식은 다제내성균 감염 환자의 관리와 고위험 환자 간호에 영향을 준다[14]. 다제내성균 감염과 관련된 인식과 실천 정도에 따른 연구를 살펴보면, 인식이 높을수록 자기평가와 실천 정도가 높았다[15]. 이것은 간호대학생의 다제내성균 감염관리에 대한 인식이 높을수록 역량도 높아질 수 있다는 것을 의미한다. 선행연구에서 다제내성균 감염관리에 대한 연구는 대상자가 간호대학생보다 간호사가 더 많았고[3,16-18], 간호대학생을 대상으로 한 감염관리 영향요인에 대한 연구도 지식, 태도, 이행에 국한된 연구가 많았다[19]. 따라서 본 연구에서는 임상실습을 경험한 간호대학생을 대상으로 그람양성균 중 가장 많은 비율을 차지하는 MRSA 감염[10]과 최근 급격히 증가하고 있는 CRE 감염[20]에 관한 감염관리 인식과 감염관리역량을 조사하고자 한다.

2. 연구의 목적

본 연구는 임상실습을 경험한 간호대학생을 대상으로 다제내성균 감염관리 인식과 감염관리역량을 확인하기 위함이며, 구체적인 목적은 다음과 같다.
  1. 대상자의 다제내성균 감염관리 인식과 감염관리역량 정도를 확인한다.

  2. 대상자의 다제내성균 감염관리 인식과 감염관리역량 간의 일치 정도를 파악한다.

  3. 대상자의 다제내성균 감염관리에 대한 인식과 감염관리역량의 상관관계를 확인한다.

연구 방법

1. 연구 설계

본 연구는 간호학과에 재학 중인 임상실습을 경험한 간호대학생을 대상으로 다제내성균 감염관리 인식과 감염관리역량을 확인하고자 시도된 서술적 조사 연구이다.

2. 연구 대상

본 연구의 대상자는 서울, 경기도, 충청남도 소재 대학 간호학과에 재학 중인 임상실습을 경험한 간호대학생이다. 예비 조사는 수정, 보완된 설문도구에 대하여 임상실습을 경험한 간호학과 3학년 재학생 15명과 4학년 재학생 15명을 대상으로 하였다. 본 조사는 표본 수 계산 프로그램인 G*Power 3.1.9.2를 이용하여 산출하였다. 상관분석을 위한 효과크기 .2, 유의수준 .05, 검정력 .8일 때 적정 표본 수는 191명이었으며, 탈락률 10%를 고려하여 210명이 산출되어 최종 240명을 대상으로 하였다.

3. 연구 도구

1) 감염관리 인식과 감염관리역량

인식이란 대상을 분별하고 판단하여 어떤 상태에 관해 명확히 알고 이해할 수 있는 것으로, 본 연구의 감염관리 인식은 다제내성균 감염관리에 대해 분별하고 판단하여 아는 것으로 정의한다. 역량이란 개인이 특정 역할을 수행하는데 영향을 주는 지식, 기술, 태도 등을 포함하는 능력으로, 본 연구의 감염관리역량은 바람직한 다제내성균 감염관리를 수행하기 위해 알아야 하는 것과 할 수 있는 것을 의미한다.

2) MRSA 감염관리 인식과 감염관리역량

Yoo 등[21]이 제작하고, Ryu와 Ko [17]가 수정한 13개의 문항을 본 연구자가 도구 사용 승인을 받은 뒤 단어 적절성, 문항 적용 가능성, 최근 변경된 지침을 고려하여 접촉주의, 상황에 맞는 손 위생, 폐기물 관리, 방문객 관리를 추가한 4점 척도의 17문항을 이용해 측정하였다. 수정된 도구가 적합한 내용인지 평가하기 위하여 전문가를 통해 내용 타당을 검정하였다. 전문가 집단은 감염관리전문간호사 자격증을 소지한 기본간호학 교수 2인, 간호대학생 임상실습 지도경력이 있는 간호학 교수 3인, 상급종합병원 감염관리실 팀장 1인으로 구성되었다. 문항 내용일치도(Item-level Content Validity Index, I-CVI)는 최소 .83에서 최대 1이었고, 문항평균 내용일치도는 .95, 총문항 내용일치도(Scale-level Content Validity Index/Universal Agreement, S-CVI/UA)는 .7이었다. 타당도 검정 이후 도구의 신뢰도 확인을 위하여 예비 조사를 실시한 뒤 2주 후 재 설문 조사하였고, 검사-재검사의 상관계수는 r=.73 (p<.001)이고, 급내상관관계(Intraclass Correlation Coefficient, ICC)는 .84 (95% CI=.66, .92)이었다.

3) CRE 감염관리 인식과 감염관리역량

CRE 감염관리 인식과 감염관리역량 도구는 미국질병관리본부(Centers for Disease Control and Prevention, CDC) 지침[22,23]과 KCDC 지침[24]을 근거로 한 Kim [25]의 문항을 재구성한 Park[18]의 21개의 문항이다. 도구 사용 승인을 받은 뒤 단어 적절성, 문항의 적용 가능성, 최근 변경된 지침을 고려하여 보호구 착용, 고위험 환자 분류, 선별 검사를 추가하였고, 접촉주의에 관한 1개의 문항을 격리해제에 관한 1개의 문항으로 수정하여 4점 척도의 24문항을 이용해 측정하였다. 수정된 도구가 적합한 내용인지 평가하기 위하여 전문가를 통해 내용 타당을 검정하였다. 전문가 집단은 감염관리전문간호사 자격증을 소지한 기본간호학 교수 2인, 간호대학생 임상실습 지도경력이 있는 간호학 교수 3인, 상급종합병원 감염관리실 팀장 1인으로 구성되었다. I-CVI는 최소 .83에서 최대 1이었고, 평균 I-CVI는 .97, S-CVI/UA는 .87이었다. 검사-재검사의 상관계수는 r=.82 (p<.001)와 ICC는 .89 (95% CI=.78, .95)이었다.

4. 자료 수집

자료 수집은 2019년 3월부터 5월까지 진행하였다. 예비 조사는 수정, 보완된 설문도구를 이용하여 임상실습을 경험한 간호대학생 30명에게 실시하고, 2주 후 재검사하였다. 본 조사는 예비 조사가 종료된 후 서울, 경기도, 충청남도 소재 간호학과에 재학 중인 임상실습을 경험한 간호대학생 210명을 대상으로 설문지를 배부하였다. 조사의 응답 시간은 약 15분 정도 소요되었고, 완성된 설문지는 직접 회수하였다.

5. 자료 분석

자료 분석은 SPSS 25.0 프로그램을 이용하였다. 응답이 불완전한 자료 9부를 제외한 나머지 201부를 최종적으로 연구 분석에 사용하였고, 예비 조사 자료 30부는 최종분석에 포함하지 않았다. 다제내성균 감염관리 인식과 감염관리역량 정도는 기술통계분석을 사용하였고, 인식과 역량 간의 일치 정도는 Kappa 지수로 확인하였다. 상관관계는 피어슨 적률상관지수(Pearson correlation coefficient)를 이용해 확인하였다.

6. 윤리적 고려

연구 수행 전에 C대학교 생명윤리위원회의 승인을 받았다 (1041078-201812-HR-226-01, 1041078-201812-HR-226-01C). 대상자를 직접 접촉하여 연구의 목적과 배경 및 절차를 설명하였고, 자발적으로 연구 참여에 동의한 경우에 한하여 서면 동의서를 받고 연구를 진행하였다. 본인이 원할 때에는 언제라도 연구 참여를 철회할 수 있다고 설명하였다. 수집된 동의서와 결과 자료는 연구 종료 후 3년 간 보관하고, 이후 안전한 방법으로 폐기할 것임을 공지하였다.

연구 결과

1. 일반적 특성

대상자는 총 201명으로 남성이 19명(9.5%), 여성이 182명(90.5%)이었고, 평균 연령은 23.2세로 20대가 많았다. 조사 시점까지 경험해본 임상실습병원의 종류와 임상실습부서의 종류는 다중응답처리를 실시하였다. 임상실습 중에 다제내성균에 감염될 가능성에 대한 인식은 10점 만점에 평균 5.36점으로 나타났다(Table 1).

2. 다제내성균 감염관리 인식과 감염관리역량

MRSA 감염관리 인식과 감염관리역량은 평균이 3.74점과 3.35점으로 감염관리역량이 더 낮은 점수로 나타났다(Table 2). MRSA 감염관리 인식에 대한 문항 중“같은 환자에게서 다른 부위를 접촉할 경우 손을 씻고 장갑을 교환한다” 가 평균 3.42점으로 가장 낮았고,“매 환자와 접촉 전, 후에 알콜젤 혹은 물과 비누를 이용하여 손을 씻는다”가 평균 3.87점으로 가장 높았다. MRSA 감염관리역량에 대한 문항 중“MRSA로 진단되면 1인실 격리 또는 코호트 격리가 필요하다”가 평균 3.02점으로 가장 낮았고,“감염의 우려가 있는 체액이나 물품과 접촉 시 일회용 장갑을 착용한다”가 평균 3.55점으로 가장 높았다(Table 3).

CRE 감염관리 인식과 감염관리역량은 평균이 3.72점과 3.28점으로 감염관리역량이 더 낮은 점수로 나타났다(Table 2). CRE 감염관리 인식과 감염관리역량에 대한 문항 중“하루에 한 번 이상 환자의 주변 물품과 병실바닥을 소독제로 닦는다”가 평균 3.57점과 3.03점으로 가장 낮았고,“환자와 접촉하기 전, 후에 손 씻기를 수행한다” 가 평균 3.90점과 3.64점으로 가장 높았다(Table 4).

3. 다제내성균 감염관리 인식과 감염관리역량 간의 일치 정도

MRSA 감염관리 인식과 감염관리역량 간의 일치 정도는 평균 Kappa값이 .21이고, 평균 일치 정도는 56.51%이었다(Table 3). CRE 감염관리 인식과 감염관리역량 간의 일치 정도는 평균 Kappa값이 .22이고, 평균 일치 정도는 54.89%이었다(Table 4).

4. 다제내성균 감염관리 인식과 감염관리역량의 상관관계

MRSA 감염관리 인식과 MRSA 감염관리역량은 상관계수가 .42로 통계적으로 유의한 양의 상관관계를 보이고 있다. MRSA 감염관리 인식과 CRE 감염관리 인식은 상관계수가 .76으로 강한 상관성을 보이고 있다. CRE 감염관리 인식과 CRE 감염관리역량은 상관계수가 .43으로 통계적으로 유의한 양의 상관관계를 보이고 있다. MRSA 감염관리역량과 CRE 감염관리역량은 상관계수가 .77로 강한 상관성을 보이고 있다(Table 5).

논 의

점점 증가하는 항생제 내성은 국내외에 심각한 건강관련문제로 거론되고 있다. 국내에서도 감염병의 예방 및 관리에 관한 법률을 개정하면서 그 경각심을 더욱더 높이고 있다. 이에 예비 간호사가 될 간호대학생을 대상으로 다제내성균 감염관리 인식과 감염관리역량을 알아보아 교육에 활용하는 것은 매우 중요하다.
MRSA 감염관리 인식에서 “같은 환자에게서 다른 부위를 접촉할 경우 손을 씻고 장갑을 교환한다”가 가장 점수가 낮게 나타났다. 이는 동일한 환자일지라도 다른 부위를 접촉할 경우에 장갑을 교환해야 한다는 주요한 지침을 인식하지 못한 것을 의미한다. 또한 다른 환자를 접촉하기 전과 감염의 우려가 있는 체액이나 물품과 접촉할 경우 손을 씻고 장갑을 교환한다는 문항들에 비해 점수가 다소 낮았다. 이 지침의 인식 저하는 간호사를 대상으로 했던 Ryu와 Ko [17]의 연구 결과와도 유사하다. 다제내성균 감염의 전파는 다제내성균에 오염되거나 감염된 환자, 보건 의료 종사자의 오염된 손 또는 환경 접촉에 의해 발생[26]되므로 올바른 손 위생과 상황에 맞는 장갑 교환을 숙지해야 할 것이다.“매 환자와 접촉 전후에 알콜젤 혹은 물과 비누를 이용하여 손을 씻는다”는 가장 높은 점수를 보였는데, 이는 Kim[12]의 연구에서도 가장 높았고, Kim 등[13]의 연구에서도“MRSA 환자 접촉 후 손 위생을 실시한다”가 세부항목 중 2번째로 높았다. 이러한 결과는 2003년에 발생된 중증급성호흡기증후군과 2015년에 발생된 중동호흡기증후군 등의 감염과 병원 내 감염 예방으로 손 위생의 중요성이 강조된 결과이다. 또한 한국간호평가원에서 실시하는 간호교육인증평가 항목 중 핵심기본간호술이 간호교육에 강조되고 있는데, 손 위생은 핵심기본간호술에서 중요한 항목이고, 대내외적으로 손 위생 교육이 활발히 이루어진 결과로 보인다. MRSA 감염관리역량에서 “MRSA로 진단되면 1인실 격리 또는 코호트 격리가 필요하다”가 가장 점수가 낮았는데, 격리에 대한 사항은 간호사를 대상으로 했던 Shon과 Park[3]의 연구에서도 3번째로 낮은 항목이었다. 이는 임상실무경험이 적은 간호대학생의 특성상 더욱이 역량이 낮았던 것으로 판단된다. 격리가 MRSA 감염 전파 감소에 직접적인 효과가 있는지는 분분하지만[27], 간접적으로는 효과가 있었다고 보고하였다[11,28]. 국내의 많은 병원은 격리를 번들 안에 포함하여 시행하고 있으므로 간호대학생의 감염관리교육에 강화되어야 할 주제이다.“감염의 우려가 있는 체액이나 물품과 접촉 시 일회용 장갑을 착용한다”는 가장 점수가 높았는데, Kim 등[13]의 연구에서도 접촉 전에 장갑을 착용해야 한다는 문항이 MRSA 관련 지식 중 가장 높았다. 본 연구에서도 해당 문항이 간호대학생의 MRSA 감염관리역량 중 가장 점수가 높았으므로 유의미한 결과이다. 장갑 착용은 MRSA 감염 전파를 약 44% 감소시킨다는 연구[29]가 있으므로, 감염관리역량을 더 향상시킬 수 있도록 반드시 강화해야 한다.
CRE 감염관리 인식과 역량에서“하루에 한 번 이상 환자의 주변 물품과 병실바닥을 소독제로 닦는다”가 가장 점수가 낮았는데, Kim[25]의 연구에서도 환경관리영역 중 가장 낮은 항목이었으므로 선행연구와 유사하였다. CDC는 다제내성균 감염관리를 위한 전략으로 소독을 포함한 환경관리를 제시하고 있다[23]. 청소가 미흡한 중환자 격리실은 철저히 청소된 경우보다 다제내성균 분리가 2배 증가했다고 보고한 연구[30]처럼 환자 주변 물품과 병실바닥의 소독은 필수적인 영역인 것이다. 특히 다른 다제내성균 감염보다 CRE의 감염 전파는 기구나 환경관리가 주요한 요인으로 작용할 수 있기 때문에 소독이 중요하다[31]. 반면에 “환자와 접촉하기 전후에 손 씻기를 수행한다”는 가장 점수가 높았는데, 이는 Park [18]의 연구에서도 가장 점수가 높은 문항이었고, Kim [25]의 연구에서도 2번째로 점수가 높은 문항이었기에 선행 연구와 유사하였다. 종합적으로 CRE 감염관리 인식과 역량은 MRSA 감염관리 인식과 역량보다 점수가 다소 낮았는데, CRE가 MRSA보다 비교적 최근에 급격히 증가하여[32] 교육이 이루어지지 못한 결과라고 보여진다.
다제내성균 감염관리 인식과 역량의 일치 정도는 낮았다. MRSA 감염관리의 경우 격리, 접촉주의 및 소독 순으로 낮았고, CRE 감염관리의 경우 소독, 격리, 접촉주의 및 보균자 색출 순으로 낮았다. MRSA와 CRE 감염관리 모두 공통적으로 격리, 접촉주의, 소독에서 일치 정도가 낮았는데, 소독의 경우 간호대학생에게 본인의 주업무가 아니라고 생각하여 도출된 결과로 보여진다. 하지만 간호보조인력의 교육을 간호사가 담당하고 있고, 또 자신의 감염관리역량을 높이기 위해서도 이 영역을 더 집중해야 할 필요가 있다. 뿐만 아니라 감염 전파는 환경표면이나 기구를 통한 전파가 잦고, 타 연구에서도 다제내성균 감염관리를 위한 전략으로 기구와 환경 소독을 권고하고 있다[31]. 의료기구의 표면은 다제내성균 전파의 잠재적 위험이 있고, 감염 전파가 지속되면 환경 청소를 강화해야 한다. 격리, 접촉주의도 다제내성균 감염관리에서 주요한 주제이며, 특히 KCDC에서 제시한 CRE 관리지침에서는 다제내성균 전파 예방 전략을 우선적으로 접촉주의와 격리로 제시하고 있다[20]. 이는 의료기관평가나 병원현장점검 시 가장 먼저 확인하는 항목이기도 하다. 따라서 간호대학생의 교육과 실습 중에 이와 같은 주제를 포함한 다양한 교육 프로그램 적용이 필요하다.
다제내성균 감염관리 인식과 감염관리역량 간 상관관계는 MRSA와 CRE 모두 중간 정도의 양의 상관관계로 나타났다. 간호대학생을 대상으로 하면서 인식과 역량에 대해 살펴본 연구가 적어 직접적으로 비교하기는 어려우나, 인식과 감염 예방에 대한 Ahn 등[33]의 연구와 인식과 감염관리 수행에 대한 Ryu 등[17], Song과 Lim [34]의 연구와 유사한 결과이다. MRSA 감염관리 인식과 CRE 감염관리 인식은 높은 상관성을 보였고, MRSA 감염관리역량과 CRE 감염관리역량도 강한 상관성을 보였다. 이는 인식이 높을수록 역량도 높아지기에 다제내성균 감염관리에 관한 교육이 학교와 병원에서 더 필요하다고 볼 수 있다. 또한 MRSA와 CRE는 다른 종류의 영역이나, 간호대학생에게 다제내성균 감염관리 원칙만 주지시킨다면 역량이 높아져 감염관리를 잘 수행할 수 있다고 판단된다. 이에 간호대학생의 역량 상승을 위한 통일된 교육 및 지침이 필요하다.
다제내성균 감염관리를 인식과 역량으로 나눈 연구결과를 통하여 간호대학생에게 적절한 교육을 제공하는 것이 무엇보다 중요하다는 것을 알 수 있다. 이번 연구결과를 통해서 간호학 교육과정과 임상에서 다제내성균 감염관리 교육을 위한 지침을 개발하고 관리하여 간호대학생들이 향후 실무에서 다제내성균 감염을 줄이기 위한 간호 활동 수행과 임상간호사로서 전문성을 확보할 수 있도록 적절한 이론과 실습기준 지침을 구체적으로 마련해야 할 것이다.
이 연구는 몇 가지 제한점을 가진다. 첫째, 감염관리역량은 알아야 하는 것과 할 수 있는 것을 모두 포함하고 있어 실제 수행과는 차이가 있다. 둘째, 다제내성균 감염관리 범위가 MRSA와 CRE로 한정되어 다제내성균 감염관리를 모두 대변하기 어렵다. 특히, VRE가 MRSA에 반코마이신 내성 유전자를 전이시킬 수 있어 VRSA를 출현하게 하는데 기여할 수 있는데 본 연구에서는 제외되었다. 마지막으로, 이 연구는 일부 학교 간호대학생을 대상으로 하였기에 일반화하기 위해서 간호학 교육과정과 실습과정을 분석하는 연구가 필요하다.

결 론

다제내성균 감염예방은 감염관리역량이 필수적으로 요구된다. 감염관리역량을 높이기 위해서는 통일된 감염관리 지침과 원칙에 근거한 교육으로 다제내성균 감염관리 인식을 높이는 것이 중요하다. 또한 감염관리역량이 증가되면 환자와 의료인은 감염노출에서 보호되고, 다제내성균 전파 차단과 환자 안전에 기여할 수 있다는 점에서 의의가 있다. 다제내성균 감염관리 인식을 높이기 위해서는 체계적인 교육을 통한 지식이 필요하고, 이를 기초로 올바른 인식이 함양되면 다제내성균 전파 예방에 기여할 수 있다. 임상실습을 통해 의료환경에 노출되는 간호대학생의 감염관리역량을 높이기 위해 간호학 교육과정에서 지식 습득과 함께 임상실습 현장에서 올바른 지침과 교육이 지속적으로 제공되어야 할 것이다.
