Iliotibial band friction syndrome (ITBFS) is defined as excessive friction between iliotibial band and lateral femoral epicondyle. It is the most common reason of lateral knee pain, especially in overuse injuries. Calcific tendinitis is defined as deposition of calcium hydroxyapatite crystals. Knee joint is a very rare site for calcific tendinitis. We are presenting a case of calcific tendinitis of distal iliotibial band as an unusual presentation of iliotibial band friction syndrome. Conservative treatment is the first option for calcific tendinitis around knee. Surgical excision is the preferred method for resistant calcific tendinitis cases. It is also the first case of calcific tendinitis of iliotibial band in the literature.
