본 연구는, 환자가 병원의 치료를 받은 후에도 치료 전의 건강 상태로 완전히 회복시키지 않는 경우가 많은 점에 주목하여, 1기간 모형을 이용하여 건강보험에서의 도덕적 해이 문제를 고찰한다. 도덕적 해이가 존재할 경우의 의료비와 보험계약의 결과를 최선(first-best)의 경우, 초과보험이나 음의 보험을 배제한 제한적 최선의 경우와 비교한다. 연구의 결과는 다음과 같다. 최선의 경우에 비해 도덕적 해이의 경우에 의료비 지출이 더 많다. 그러나, 건강손실이 큰 경우에는 도덕적 해이에 따른 의료비가 제한적 최선의 경우보다 더 적은 경우도 발생할 수 있다. 한편, 건강손실이 작은 경우에는 도덕적 해이가 있을 때의 보험보호율이 다른 경우보다 더 커져야 할 수도 있음을 관찰하였다. 이 결과는 건강보험의 설계에 있어서 도덕적 해이의 해결 방안과 관련된 시사점을 제공해 준다.
We consider a one-period model of moral hazard when the insured (patient) may not fully recover their health status after treatment. We compare the treatment costs and the insurance contract under moral hazard with those of the first-best case and of the constrained first-best in which insurance coverage is confined between zero and one. Our main finding is that while moral hazard leads to higher treatment costs than the first-best case, it can be lower than the constrained first-best case when the health loss is large. When the health loss is small, insurance coverage under moral hazard can be higher than those of the first-best and the constrained first-best case. These results provide an policy implication on the design of health insurance.
