Platelet distribution width as a novel inflammatory marker for fibromyalgia

Öz



Objective:
Fibromyalgia syndrome (FMS) is a benign condition that is characterized by is chronic widespread pain at multiple tender points, systemic
symptoms such as fatigue, cognitive dysfunction and insomnia.
The
aim of this study was to investigate the relationship between FMS and platelet
distribution width (PDW), red cell distribution width (RDW), mean platelet
volume (MPV), neutrophil to lymphocyte ratio (NLR), platelets to lymphocyte
ratio (PLR) and other haematological parameters.

Material and
Methods:
The
patient group consisted of 200 FMS patients who had met the
American
College of Rheumatology (ACR) classification
criteria for FMS, and the control group consisted of 65 healthy volunteers.
Carefully
obtained history, detailed physical examination, hematologic and biochemical tests
were performed. The groups were investigated by comparison and correlation
analyzes.

Results: In the FMS
group PDW was
17,1±1,9; RDW was 12,5±1,4; MPV was 8,3±1,2
fl; NLR was 1,81±0,67 and PLR was 114,1±32,4. For the
control group, the results were as; PDW
17,6±2,1;
RDW 12,3±1,2; MPV 8,5±1,4; NLR 1,84±1,11 and PLR 111,9±38,6. There was
no difference between the two groups for RDW, MPV, NLR and PLR. PDW was
statistically significantly lower in FMS group than controls (p <0.05).

Conclusions: PDW may
play a role as an early inflammatory marker for the pathogenesis and diagnosis
of FMS
.

Fibromiyalji tanısında inflamatuar bir belirteç: Platelet dağılım genişliği

Öz

Amaç: Fibromiyalji sendromu (FMS), çoklu
hassas noktalarda kronik yaygın ağrı ile karakterize, yorgunluk, uykusuzluk,
kognitif disfonksiyonlarda bozukluk vb. sistemik semptomlarla bir arada
seyreden bir sendromdur.
Bu çalışmanın amacı, FMS tanılı
hastalar ile kontrol grubu arasındaki platelet dağılım genişliği (PDW), kırmızı
küre dağılım genişliği (RDW), ortalama trombosit hacmi (MPV), nötrofil/lenfosit
oranı (NLR), platelet/lenfosit oranı (PLR) hematolojik parametrelerini
araştırmak ve hastalık etyopatogenezine katkı sağlamaktır.



Gereç ve Yöntem: Fizik
Tedavi ve Rehabilitasyon polikliniğine ayaktan başvuran ve
‘American College of Rheumatology’ (ACR) kriterlerine göre FMS tanısı
konan 200 hasta ve 65 sağlıklı birey çalışmaya dahil edildi. Muayeneleri
kapsamında ayrıntılı anamnez alımı, detaylı fizik muayene, hematolojik ve
biyokimyasal parametreleri incelendi. İstatistiksel olarak karşılaştırma ve
analizleri yapıldı.



Bulgular: FMS
grubunda PDW
17,1±1,9, RDW 12,5±1,4; MPV 8,3±1,2
fentolitre (
fl); NLR 1,81±0,67; PLR 114,1±32,4
idi. Kontrol grubunun ise PDW değeri 17,6±2,1 (p=0,034);
RDW
12,3±1,2; MPV 8,5±1,4; NLR 1,84±1,11; PLR 111,9±38,6 idi. Yapılan
karşılaştırmalarda RDW, MPV, NLR, PLR de her iki grup arasında fark
saptanmazken; FMS tanılı hastalarda PDW, kontrol grubuna göre istatistiksel
olarak anlamlı düzeyde düşük saptandı (p<0.05).



Sonuç: PDW’nin, FMS patogenezinde muhtemel inflamatuar aktivitenin hematolojik
parametrelerdeki göstergesi olarak ve FMS nin erken tanısında bir belirteç
olarak rolü olabilir.
