Als Wadenkrämpfe werden alle abrupt auftretenden, unwillkürlichen, schmerzhaften Kontraktionen eines Teils oder der gesamten Wadenmuskulatur bezeichnet, die sichtbar sind, Sekunden bis Minuten anhalten und danach spontan wieder abklingen. Sie können sporadisch auftreten und werden in diesen Fällen als gewöhnliche Wadenkrämpfe bezeichnet. Symptomatisch sind sie bei Erkrankungen des peripheren und des zentralen Nervensystems zu beobachten. Weiterhin treten sie bei Krankheiten auf, die mit metabolischen Störungen einhergehen. Die Krämpfe sind dann umfassender, intensiver und länger anhaltender. Das Faszikulations-Krampus-Myalgie-Syndrom ist darüber hinaus durch Parästhesien und andere Zeichen einer Hyperexzitabilität peripherer Nerven gekennzeichnet. Therapeutisch wird bei entsprechendem Leidensdruck eine orale Gabe von Chinin und/oder die Behandlung der Wadenmuskeln mit Botulinumtoxin empfohlen. In der Schwangerschaft sind beide Präparate kontraindiziert, eine probatorische Magnesiumgabe dagegen ist indiziert.

Summary

Calf cramps are defined as abrupt, involuntary and painful contractions of the whole calf musculature, or parts of it. They are visible, can last a few seconds to minutes, and will then subside spontaneously. These cramps can appear sporadically and, in that case, are called common calf cramps. In diseases of the peripheral and the central nervous system they appear to be symptomatic. Furthermore, they occur in illnesses accompanied with metabolic disorders. Then, the cramps are more extensive, more severe and will last longer. Moreover, the cramp-fasciculation-myalgia-syndrome is characterized by paresthesias and other signs of hyperexcitability of peripheral nerves. According to the level of pain, an oral dose of quinine and/or botulinum toxin treatment is suggested. During pregnancy both preparations are contraindicated; therefore, probationary doses of magnesium are advisable.
