Modified Barcat Technique in Distal Hypospadias Repair

Abstract

Aim: A modified Barcat operation, when carried out properly, provides an anatomically and cosmetically successful result in distal hypospadias repair in each glans configuration. We retrospectively evaluated our experience with this technique in the last five years.

Patients and Methods: 78 children with primary distal type hypospadias were operated between 1998 - 2003, using a modified Barcat technique.

Results: The mean age of the patients was 3 years (6 months - 14 years). The mean follow-up period was 2 years (1 month - 60 months). Nesbit plication was performed in four cases with chordee. Fistula developed in five (6.5 %) cases and required reoperation. Stenosis which did not require surgical intervention occurred in 5 children and resolved with self-dilatations.

Conclusion: The Barcat technique is an appropriate operation for distal type hypospadias cases but should be modified by creating second tissue layers over the neourethra and a wide meatus.

But: La technique modifiée de Barcat, quand elle est bien indiquée, entraîne un résultat anatomique et esthétique satisfaisant dans la réparation des hypospadias distaux. Nous évaluons de manière rétrospective, notre expérience avec cette technique dans les cinq dernières années.

Patients et Méthodes: 78 enfants avec un hypospadias distal étaient opérés de 1998 à 2003 avec la technique de Barcat modifiée.

Résultats: L'âge moyen des patients était de 3 ans (6 mois - 14 ans). La durée moyenne du suivi était de 2 ans (1 mois - 60 mois). Une plicature selon Nesbit était réalisée dans quatre cas. Une fistule se développait dans cinq (6,5 %) des cas et nécessitait une ré-opération. Une sténose qui ne nécessitait pas une reprise chirurgicale était observée chez cinq enfants et se traitait par des auto-dilatations.

Conclusion: La technique de Barcat est tout à fait appropriée pour le traitement des hypospadias distaux mais elle doit être modifiée par la création d'un second plan au-dessus du néo-urètre.

Objetivo: Cuando se realiza adecuadamente una operación de Barcat modificada en el hispospadias distal se consigue un resultado anatómico y cosmético excelente en cuanto a la posición del meato y a la configuración del glande. Evaluamos retrospectivamente nuestra experiencia con esta técnica en los últimos 5 años.

Pacientes y Métodos: Operamos con la técnica modificada de Barcat a 78 niños con hipospadias distal primario entre 1998 y 2003.

Resultados: La edad media de los pacientes fue de 3 años (rango 6 meses a 14 años). El seguimiento medio fue de dos años (rango 1 mes a 60). Se realizó plicatura de Nesbitt en cuatro casos que tenían cuerda ventral. Se observó fístula en 5 (6.5 %) de los casos que requirieron reoperación. Ocurrió estenosis que no necesitó intervención quirúrgica en 5 niños que se solucionó con autodilataciones.

Conclusión: La técnica de Barcat es una operación adecuada para el hipospadias distal que debe ser modificada creando otras capas secundarias de tejido sobre la neouretra y especialmente ampliando el meato.

Zusammenfassung

Die sorgfältig ausgeführte, modifizierte Barcat-Technik führt zu einem anatomisch und kosmetisch ausgezeichneten Ergebnis bei distalen Hypospadien. Die Glans kann physiologisch rekonfiguriert werden. In der vorliegenden Arbeit werden die Ergebnisse der vergangenen fünf Jahre dargestellt.

Patienten und Methodik: 78 Kinder mit einer distalen Hypospadie wurden im Zeitraum von 1998 bis 2003 mit der modifizierten Barcat-Technik operiert. Das Durchschnittsalter der Patienten lag bei 3 Jahren (6 Monate bis 14 Jahre). Die mittlere Beobachtungszeit lag bei 2 Jahren (1 Monat bis 60 Monate). Bei 4 Kindern wurde eine Nesbit-Operation durchgeführt, diese Patienten hatten auch eine Chorda. Fisteln entwickelten sich bei 5 Kindern (6,5 %) und bedingten eine Reoperation. Fünf weitere Knaben entwickelten eine Stenose, die jedoch nicht reoperiert werden musste. Mit Selbstdilatationen konnte das Problem behoben werden.

Schlussfolgerung: Die Barcat-Technik ist eine gute und für distale Hypospadieformen geeignete Operationsmethode. Sie sollte jedoch dadurch modifiziert werden, dass eine zweite Gewebsschicht über die Neourethra und insbesondere den weiten Meatus gelegt wird.
