Paciente de 16 años con lupus eritematoso sistémico en tratamiento con HCQ (Dolquine®) 200 mg/día durante 8 años. Enviado por su pediatra para control oftalmológico rutinario. Discusión
La HCQ es un antimalárico que se utiliza por su acción antiinflamatoria. La HCQ tiene absorción gastrointestinal rápida y eliminación renal. Se une fuertemente a los tejidos con melanina2. La HCQ ha reemplazado a la cloroquina por su menor toxicidad ocular a dosis elevadas, aumentando bruscamente después de 5 a 7 años o de una cantidad acumulada de 1.000 g de HCQ. SD-OCT changes such as a reduction in retinal thickness, increased loss of photoreceptors or retinal pigment epithelium, marked discontinuity of perifoveal photoreceptor junctions between the inner and outer segments and thinning of the outer nuclear layer development or increase in cystoid macular edema, and development of epiretinal membranes have been reported in patients treated with hydroxychloroquine over a prolonged period [6,12,13,24]. Measurement of macular thickness using SD-OCT is a reliable and reproducible method with a low coefficient of variation in repeated measurements [25].
