The identification of comorbidities was based on providers' diagnoses for the visit, responses to questions regarding presence of comorbidities, medications prescribed, and other methods described elsewhere.17 Since 2005, in addition to the 3 primary diagnoses, physicians were interviewed on 14 yes/no questions regarding presence of certain comorbid conditions, including arthritis, asthma, cancer, cerebrovascular disease, congestive heart failure, chronic renal failure, chronic obstructive pulmonary disease, depression, diabetes, hyperlipidemia, hypertension, ischemic heart disease, obesity, and osteoporosis.

Patients with CKD-associated anemia were identified by ICD-9-CM codes for anemia (including 281.0, 281.1, 281.2, 281.8, 283, 284.0, 285.1, 285.21, 285.9) or medication codes for erythropoietin, darbepoetin, or iron prescription, or patients that were ordered pertinent laboratory test (e.g., Hgb, hematocrit) and ICD-9-CM codes for CKD or medication codes for CKD patient specific medications were present (Rasu et al., 2007). The medications prescribed are assigned with a unique NCHS-assigned, 5-digit code applied to each entry name mentioned in NAMCS data set.
