La actual epidemia de enfermedad por el virus Ébola ha establecido la necesidad de desarrollar algún tipo de vacuna efectiva frente a este virus. La utilización de la misma permitiría proteger no solo a la población de elevado riesgo (personal sanitario), sino a toda la población sin contacto previo con el virus.
Frente al Ébola existen 2 tipos de vacunas, las no replicativas (inactivadas, proteínas de fusión, replicones y de ADN) que facilitan la interacción entre el antígeno viral (la glucoproteína GP) y el sistema inmune. Las vacunas replicativas se basan en la inserción del gen GP del virus Ébola en diferentes vectores virales replicativos. Así se han utilizado inicialmente como vectores el virus de la parainfluenza, el de la rabia y el citomegalovirus. Sin embargo, las 2 vacunas recombinantes que han mostrado mayor eficacia protectora utilizan adenovirus recombinantes (serotipos 3 y 5) y el virus de la estomatitis vesicular recombinante.
Ambos tipos de vacunas parecen inducir preferentemente inmunidad humoral y/o celular, pero parecen ser las vacunas del futuro. Especialmente la vacuna cAd3, que en un ensayo clínico reciente en fase i, ha mostrado una escasa reactogenicidad y una intensa inmunogenicidad, aunque dependiente de la dosis.
La existencia futura de estas vacunas obliga a plantearse cuál será la mejor estrategia vacunal y quién se hará cargo de los costes de administración y distribución.
The current epidemic of Ebola virus disease has established the need to develop some kind of effective vaccine against this virus. The use of the vaccine would protect not only the high-risk population (health workers) but also all those people without previous contact with the virus.
Vaccines against Ebola virus are classified in two types, non-replicating (inactivated, fusion proteins and DNA replicons) that facilitate interaction between viral antigen (glycoprotein GP) and the immune system. Replicative vaccines are based on the insertion of Ebola virus GP gene in different replicative viral vectors. So were initially used as vectors parainfluenza virus, the rabies and cytomegalovirus. However the two recombinant vaccines have shown greater protective efficacy are recombinant adenovirus (serotypes 3 and 5) and recombinant virus vesicular stomatitis.
Both types of vaccines appear to preferentially induce humoral and/or cellular, but would be vaccines of the future. CAd3 vaccine especially in a recent phase i clinical trial has shown poor reactogenicity and intense immunogenicity, but dose-dependent.
The future existence of these vaccines must raise what is the best vaccination strategy and who will pay for the costs of administration and distribution.
