Response to cytarabine in progressive multifocal leucoencephalopathy in AIDS

The symptoms associated with slow viral or prion diseases of the central nervous system tend to have multiple neurologic symptoms, and different patients may present with different symptoms. This review discusses the most common slow virus infections and their imaging findings.

La leucoencéphalopathie multifocale progressive (LEMP), causée par le virus JC (VJC), survient le plus souvent dans un contexte de déficit de l’immunité cellulaire et plus rarement de l’immunité humorale.

Dans ce travail, nous décrivons deux groupes de patients Le premier concerne trois patients atteints de lymphomes de type B qui développèrent une LEMP, confirmée histologiquement Le second groupe comprend six patients séropositifs pour le VIH ayant présenté une LEMP.

Dans le premier groupe, la survenue de la LEMP était principalement associée à un déficit sévère de l’immunité humorale (taux des IgG inférieur à 4 g/l) Le taux de lymphocytes T-CD4+ était supérieur à 200/mm³ La survie moyenne de ces patients fut de 5 mois. Dans le second groupe, comportant des patients. atteints par le VIH, la LEMP était contemporaine d’une immunodépression cellulaire sévère avec une lymphopénie voisine de 50/mm³ en moyenne Leur durée de survie était comprise entre 2 et 58 mois, et l’un d’eux est actuellement en vie.

Chercher à réduire le déficit immunitaire reste primordial, mais est-il seul en cause dans le déclenchement et le développement de la LEMP?.

Progressive multifocal leukoencephalopathy (PML) caused by JC virus (JCV) mostly occurs in different conditions of impaired cellular immunity like acquired immunodeficiency syndrome (AIDS) and rarely when humoral immunity is involved PML remains unusual although there is a high prevalence of JCV among the population and immunosuppression is not rare because of chemotherapies.

We present two groups of patients: first, we studied reports of three patients suffering from lymphoma type B who developed a PML, proved by cerebral biopsy The second group included six HIV-infected patients who developed a PML No biopsy was made but MRI and the physical examination suggested strong arguments for the diagnosis.

In the first group, PML was furthered by humoral immunosuppression (rate of immunoglobulin G under 4 g/l). Average survival was five months. In the second group, HIV-infected patients had a survival range from 2 to 58 months after the first PML symptoms and one of them is still alive.

Humoral immunosuppression in lymphoma can contribute to the development of PML PML prognosis is often severe but prolonged survivals were described. So it is necessary to restore a sufficient immunity level. But immunity failure may be insufficient to lead to PML. In the case of lymphomas, the role of malignant lymphocytes in multiplication and mutation of JCV might be an interesting pathophysiological hypothesis.
