Hintergrund

Die Insuffizienz des vorderen Kreuzbandes (VKB) führt nicht nur zu einer subjektiven Instabilität des Kniegelenks, sondern auch zu chronischen Fehlbelastungen der Menisken und des Knorpels.

Ziel der Arbeit

Das Ziel der vorliegenden Arbeit war, den Einfluss der VKB-Insuffizienz auf sekundäre Meniskus- und Knorpelschäden aufzuarbeiten. Zudem sollte der protektive Effekt der VKB-Rekonstruktion auf diese degenerativen Schäden erörtert werden.

Material und Methoden

Es wurde eine systematische Literatursuche entsprechend den PRISMA-Kriterien durchgeführt. Eingeschlossen wurden Studien, die die Rate sekundärer Meniskus- und Knorpelschäden zu unterschiedlichen Zeitpunkten nach der initialen VKB-Ruptur angaben. Des Weiteren wurde nach Studien gesucht, die die Inzidenz von Meniskus- und/oder Knorpelschäden mit und ohne VKB-Rekonstruktion nach stattgehabter VKB-Ruptur verglichen haben.

Ergebnisse

In 84,2 % bzw. 85,3 % der eingeschlossenen Studien konnte eine positive Korrelation zwischen der Zeit seit der VKB-Ruptur und der Rate an sekundären Meniskus- bzw. Knorpelschäden festgestellt werden. Für den Innenmeniskus lag ein 2‑fach erhöhtes Risiko bereits 6 Monate nach der VKB-Ruptur vor. Für sekundäre Knorpelschäden war das Risiko bereits nach 12 Monaten 2,4-fach erhöht. Die Studienlage zum protektiven Effekt der VKB-Rekonstruktion ist sehr heterogen, jedoch liefert der überwiegende Teil der Studien Evidenz dafür, dass die VKB-Rekonstruktion das Risiko für Knorpel- und Meniskusschäden sowie für die Entwicklung einer posttraumatischen Gonarthrose senken kann.

Diskussion

Die chronische VKB-Insuffizienz führt bereits nach wenigen Monaten zu sekundären Schäden an Menisken und Gelenkknorpel. Die Literatur liefert Evidenz dafür, dass die VKB-Rekonstruktion einen protektiven Effekt auf die Entstehung dieser degenerativen Schäden hat.

Abstract

Background

Insufficiency of the anterior cruciate ligament (ACL) is not only associated with subjective knee instability but it also leads to chronic overload of the menisci and the cartilage.

Objective

The aim of the present study was to investigate the influence of ACL insufficiency on the development of secondary meniscal and cartilage lesions. Additionally, the protective effect of ACL reconstruction on these degenerative injuries was discussed.

Material and methods

A systematic literature search was performed according to the PRISMA guidelines. Articles comparing the rate of secondary meniscal and cartilage lesions diagnosed at different time points of ACL reconstruction were included. The search also included studies comparing the rate of secondary meniscal and/or cartilage lesions between patients following ACL reconstruction and conservative treatment.

Results

In 84.2% and 85.3% of the included studies there was evidence for a positive correlation between the time since the ACL rupture and the rate of meniscal and cartilage lesions, respectively. The risk for medial meniscal injuries was doubled after 6 months. For secondary cartilage lesions the risk was 2.4-fold 12 months after the initial ACL injury. The current literature shows a relevant heterogeneity regarding the protective effect of ACL reconstruction; however, the majority of the studies showed evidence for a positive influence of ACL reconstruction on the development of meniscal and cartilage lesions as well as on the development of posttraumatic osteoarthritis of the knee.

Conclusion

Chronic deficiency of the ACL leads to secondary meniscal and cartilage lesions even after a few months. There is evidence that ACL reconstruction has a protective effect on the development of these degenerative injuries.
