Zusammenfassung
Hintergrund
Hinsichtlich der Sekundärprävention wird derzeit trotz gesteigerter antihypertensiver Medikation ein unverändert hoher Blutdruck bei Patienten mit koronarer Herzkrankheit postuliert. Zeigt sich dies auch im Disease-Management-Programm (DMP) koronare Herzkrankheit (KHK) Nordrhein?
Material und Methoden
Im Jahr 2008 waren 186.599 Patienten mit gesicherter koronarer Herzkrankheit (KHK) in das DMP eingeschrieben (63,2% männlich, Alter 70,2±10,3 Jahre, Erkrankungsdauer 8,2±6,7 Jahre). Messwiederholungsvarianzanalyse der Blutdruckveränderung von 2004–2008 und logistische Regressionsanalyse (systolischer Blutdruck <130 mmHg 2008).
Ergebnisse
Im Gesamtkollektiv veränderte sich der systolische Blutdruck abhängig vom Ausgangswert um −29,4 bis +5,4 mmHg. Bei Patienten mit Herzinfarkt oder Herzinsuffizienz wurden 129 (95%-KI=128,5–129,6) bzw. 129,2 (95%-KI=128,7–129,8) mmHg erreicht (Odds-Ratio für <130 mmHg 1,15; 95%-KI=1,12–1,17 bzw. 1,19; 95%-KI=1,17–1,23). Parallel nahm die kombinierte Verordnung von Betablockern und ACE-Hemmern zu, davon profitierten vorrangig jüngere männliche Patienten.
Schlussfolgerung
Die Blutdrucksenkung bei KHK-Patienten war insgesamt erfolgreicher als erwartet, in einzelnen Patientengruppen (Frauen, Diabetiker) kann sie jedoch weiter intensiviert werden.
Abstract
Background
With respect to secondary prevention it is postulated that despite increased antihypertensive medication the blood pressure of patients with CAD nowadays remains high. Is this also true for the DMP CAD in North Rhine?
Material and methods
By 2008 186,599 patients with confirmed CAD took part in the DMP (63.2% male, age 70.2±10.3, CAD duration 8.2±6.7 years). Use of repeated measurements analysis of changes in blood pressure from 2004 until 2008 and logistic regression analysis (systolic blood pressure < 130 mmHg 2008).
Results
Dependent on the initial value, systolic blood pressure in the total sample differed by –29.4 up to +5.4 mmHg. Patients with myocardial infarction or heart failure ended up with 129 (95% CI: 128.5–129.6) and 129.2 (128.7–129.8) mmHg, respectively [odds ratio for < 130 mmHg 1.15 (1.12–1.17) and 1.19 (1.17–1.23)]. Combined prescription of beta blockers and ACE inhibitors increased; younger male patients profited from that to a larger degree.
Conclusion
In general blood pressure reduction in patients with CAD was more successful than expected, but in some patient groups (women, patients with diabetes) it can be further intensified.
