Findings of parotid basal cell adenoma on magnetic resonance imaging

Abstract

Purpose

To explore the features of basal cell adenoma of the parotid gland on magnetic resonance imaging (MRI).

Methods

The MRI features of 12 parotid basal cell adenomas (6 females, 6 males; mean age 53 years) confirmed by pathology were retrospectively analyzed. All MR images were evaluated with respect to tumor number, size, location, shape, marginal morphology, presence of necrosis/cysts, and enhancement behavior.

Results

All cases showed a single mass, giving a total of 12 masses in the 12 cases. Seven masses were located in the left parotid and five masses in the right parotid. Nine masses involved the superficial and deep parotid lobes simultaneously. Regarding shape, seven masses were oval and five were round. The maximum diameter of the mass was <3 cm in 10 cases. All masses had a capsule and well-defined margin. A necrotic/cystic region was found in all masses. The solid regions of all masses showed hypointensity on T1-weighted images (T1WI)/T2-weighted images (T2WI) and hyperintensity on coronal fat suppression T2WI compared with surrounding parotid tissues. The solid regions of 11 masses showed slight heterogeneous enhancement, while the remaining tumor with >90 % cystic component was not analyzed for homogeneity of enhancement. All masses showed significant enhancement in the arterial phase and 8 masses showed plateau-type enhancement in the venous phase.

Conclusions

Typical basal cell adenomas in the parotid appear as oval/round masses with a well-defined margin, focal cystic/necrotic component, hypointensity on T1WI/T2WI compared with surrounding parotid tissues, mild heterogeneous internal enhancement, significant arterial enhancement, and plateau-type venous enhancement.
