Zusammenfassung
Hintergrund
Penetrierende Thoraxverletzung stellen eine notfallmedizinische und -chirurgische Herausforderung dar. Neben fundierten pathophysiologischen Kenntnissen sind häufig bereits präklinisch invasive Maßnahmen und weitreichende chirurgische Fähigkeiten, bis hin zur Anlage einer Thoraxdrainage und Perikardiozentese vor Ort, notwendig.
Methode und Ergebnisse
In dieser Kasuistik wird über das erfolgreiche Management eines wachen, spontan atmenden Patienten, welcher sich im Rahmen schizophrener Wahnvorstellungen eine offene Thoraxverletzung selbst zugefügt hatte, berichtet. Neben dem Fallbericht werden die wesentlichen diagnostischen und therapeutischen Maßnahmen diskutiert. Dieser Fall zeigt das erfolgreiche Management eines offenen Pneumothorax bei spontan atmenden Patienten mittels nichtokklusiven Verband.
Schlussfolgerung
Aufgrund der respiratorischen Stabilisierung nach dieser Maßnahme konnte Zeit zugunsten eines schnellen Transports und frühen definitiven chirurgischen Versorgung gespart werden.
Abstract
Background
Penetrating thoracic injuries are challenging in preclinical and clinical settings. Beside profound physiological knowledge of the respiration, invasive surgical measures often are needed. In extreme situations, on scene chest-tube application or pericardiocentesis might be necessary. Our case represents a successful management of an awake, spontaneously breathing patient with a self-mutilated open pneumothorax during schizophrenic hallucinations.
Method and results
The main diagnostic and therapeutic measures and surgical skills demanded are presented and discussed. Our case underlines the possibility of a successful resuscitation of an open pneumothorax in spontaneously breathing patients by a non-occlusive thoracic dressing.
Conclusion
Due to respiratory recompensation, intubation was not necessary and therefore, valuable time could be saved in favor of a quick admission to a trauma center.
