Zusammenfassung
Australien und Neuseeland weisen eine reichhaltige und langdauernde Tradition des Hospizwesens und der Palliativmedizin auf. Ursprünglich waren es christliche Ordensschwestern, die Hospizeinrichtungen gründeten. Heute wird eine zentral gesteuerte Koordination auf der Grundlage einer evidenzbasierten Praxis und einer dem Bevölkerungsbedarf entsprechenden Finanzierung angestrebt. Aktuelle Herausforderungen bestehen in einer schnell alternden Bevölkerung, die an einer Vielzahl chronischer nichtmaligner Erkrankungen leidet, in der Sicherung einer adäquaten Finanzierung und in der Auseinandersetzung um Sterbehilfe und ärztlich assistierten Suizid. In Down Under, ebenso wie in Deutschland, ist eine zukunftsorientierte einheitliche Strategie erforderlich, die unter Mitbestimmung aller Palliativmedizin- und Hospizeinrichtungen das Ziel hat, die Gleichwertigkeit von medizinischer Behandlung und Palliation anzuerkennen, die Palliativmedizin in das gesamte Gesundheitssystem einzubinden, Palliativbetreuung in Alters- und Pflegeheimen zu fördern und den kulturell sensiblen Umgang mit ethnischen Minderheiten zu garantieren.
Abstract
Australia and New Zealand have a long and rich tradition of hospice care and palliative medicine. Hospices were originally founded by Christian nuns. Today, being centrally coordinated, care is founded on evidence-based principles and funding is divided to meet the needs of the people served. Current challenges include the rapidly aging population which suffers from a variety of chronic nonmalignant diseases, the assurance of adequate financing, and the debate on euthanasia and physician-assisted suicide. In Germany as in Down Under, a future-oriented uniform strategy is necessary in order to ensure that palliative care —with participation from all palliative medicine and hospice programs —is recognized as an integral part of the healthcare system, is promoted in aged and nursing home care, and is delivered to ethnic minorities in a culturally sensitive.
