Hintergrund
Das Fibromyalgiesyndrom (FMS) stellt für die Krankheitsbehandlung eine große Herausforderung dar. Dabei scheint v. a. eine komorbide Depression die Symptome und die damit einhergehenden Beeinträchtigungen negativ zu beeinflussen. Die vorliegende Arbeit gibt einen systematischen Überblick über den Einfluss der Depressivität auf physische und psychische Beeinträchtigungen, die Krankheitsbewältigung und den Behandlungserfolg bei Vorliegen eines FMS. Es wurden veröffentlichte Studien aus den Datenbanken Pubmed, Embase und PsychINFO bis April 2010 berücksichtigt.
Ergebnisse und Schlussfolgerungen
FMS-Patienten mit Depression geben mehr Schlafstörungen, sexuelle Dysfunktion, körperliche Leistungseinschränkungen und eine niedrigere Lebensqualität an. Sie wenden eher passive Bewältigungsstrategien an und neigen zur Katastrophisierung. Patienten ohne komorbide Depression zeigen einen größeren Behandlungserfolg durch eine multimodale Rehabilitation als Patienten mit Depression. Aus diesem Grund sollte eine Erhebung von Depression in die routinemäßige Diagnostik des FMS mit aufgenommen werden. Zudem sollten depressive Patienten mit FMS nachhaltig zur Behandlung motiviert werden und Unterstützung bei der Bewältigung ihrer Depression erhalten.
Abstract
Background
Fibromyalgia poses a challenge for therapy. Mainly comorbid depression has a negative impact on symptoms and impairment in patients with fibromyalgia. This paper gives a systematic overview of how depressiveness affects physical and psychological impairment, coping with disease and rehabilitation outcome among patients with fibromyalgia. The databases PubMed, Embase and PsychINFO were searched for studies published up to April 2010.
Results and conclusions
Fibromyalgia patients with depressive symptoms show more sleep disturbances, sexual dysfunction, loss of physical function level and lower quality of life. Patients with fibromyalgia and comorbid depression showed poorer outcomes of multimodal rehabilitation than patients with fibromyalgia without depression. Therefore the assessment of depression should be included in the routine diagnostics of fibromyalgia. Furthermore, patients with fibromyalgia and comorbid depression should be motivated to seek therapy and get better support in coping with depression.
