Zusammenfassung
Hintergrund
Insbesondere bei komplizierten Aortenklappenendokarditiden oder Infektionen im Bereich der Aorta durch mykotische Aortenaneurysmen und Protheseninfektionen sowie im Rahmen der Ross-Operation ist der Einsatz von allogenen Herzklappentransplantaten weiterhin indiziert. Die Herstellung solcher Allografts ist in Deutschland Aufgabe von kardiovaskulären Gewebebanken.
Material und Methoden
Im Rahmen einer Tätigkeitsanalyse der Jahre 2007 bis 2010 wurden die Eckdaten zu Spenderzahlen, Herstellung und Abgabe sowie den technischen Bedingungen der beteiligten vier kardiovaskulären Gewebebanken (Bad Oeynhausen, Berlin, Kiel, München) und als externe Referenz zusätzlich die Kennzahlen der KVGB Rotterdam erfasst.
Ergebnisse
Die vier deutschen kardiovaskulären Gewebebanken haben durchschnittlich 44 Aortenklappen- und 95 Pulmonalklappen-Allografts pro Jahr an die klinischen Anwender ausgeliefert. Unter Hinzuziehung der jährlich importierten Klappen-Allografts dürfte der durchschnittliche Bedarf in Deutschland bei etwa 220 Herzklappen-Allografts pro Jahr liegen. Das Ausgangsgewebematerial wurde bei jährlich etwa 100 Multiorganspendern, 45 Herz-Kreislauf-Toten und 80 Dominospendern gewonnen.
Diskussion
Die beteiligten kardiovaskulären Gewebebanken verfügen über vergleichbare technische und administrative Voraussetzungen und sind in der Lage, nach den Regeln der guten fachlichen Praxis gemäß § 3 (3) AMWHV Gewebezubereitungen herzustellen und deren Qualität zu bewerten, wobei eine Harmonisierung des mikrobiologischen Monitorings bzw. der Antibiose noch erfolgen muss.
Abstract
Background
Especially in complicated aortic valve endocarditis, infections of the aorta by mycotic aortic aneurysms and prosthetic infections, or as part of the Ross procedure, the use of allogeneic heart valve transplants remains important. The production of such allografts in Germany is the task of cardiovascular tissue banks (CVTB).
Materials and methods
During an analysis of the years 2007–2010, basic data on donor numbers, production, and distribution as well as the technical conditions of not only the four participating CVTB (Bad Oeynhausen, Berlin, Kiel, Munich) but also data from the CVTB Rotterdam as an external reference were recorded.
Results
The German CVTB delivered an average of 44 aortic and 95 pulmonary allografts per year to clinical users. By incorporating the annually imported valve allografts, the demand in Germany approximately averages 220 heart valve allografts per year. The heart tissue was harvested from approximately 100 multiorgan donors, 45 cardiovascular deaths, and 80 domino donors annually.
Discussion
The participating cardiovascular tissue banks have comparable technical and administrative requirements and are able to produce tissue preparations according to the rules of Good Professional Practice in accordance with § 3 (3) AMWHV to assess their quality, whereby harmonization of microbiological monitoring and antibiotic treatment is still necessary.
