Association of angiographic and immunohistochemical findings in graft vessel disease after heart transplantation

Zusammenfassung

Hintergrund

Die Transplantatvaskulopathie (TVP) ist eine der hauptlimitierenden Faktoren für das Langzeitüberleben nach Herztransplantation (HTx). Ziel der Studie war zu untersuchen, ob es einen Zusammenhang zwischen Veränderungen der terminalen Strombahn und angiographischen Befunden graduiert anhand der Stanford Klassifikation und dem Gensini Score gibt.

Material und Methoden

Es wurden 30 Patienten (8 Frauen, 22 Männer, mittleres Alter bei HTx 48 Jahre) untersucht. Von 209 rechtsventrikulären Rejektionskontrollbiopsieproben (EMB), die während der ersten 14 Monate nach HTx gewonnen wurden, wurden immunhistochemische Untersuchungen mit Antikörpern gegen α-Aktin (glatte Muskelzellen, SMCs) und Faktor (F) VIII (250 000 kDa, v. Willebrand-Faktor in Endothelzellen) durchgeführt. Von den Patienten wurden jeweils die erste und letzte koronarangiographische Untersuchung (mittlere Zeit nach HTx 8 und 44 Monate) evaluiert. Gemäß ihrer koronarangiographischen Befunde, graduiert anhand der Stanford-Klassifikation und dem Gensini Score, wurden die Patienten in eine TVP- und eine Vergleichsgruppe unterteilt. Alle Ergebnisse wurden anhand des χ2 und Mann-Whitney-Tests analysiert.

Ergebnisse

1. Alle Patienten mit angiographischem Nachweis höhergradiger Koronararterienstenosen zeigten signifikant mehr α-Aktin- und F VIII-positive Blutgefäße (p < 0,05). 2. Es fand sich eine positive Korrelation von α-Aktin- und F VIII-positiven Blutgefäßen mit dem Auftreten von Stanford-Läsionen genauso wie der Höhe des Gesamtscores nach Gensini (p < 0,05). 3. Bei Patienten mit Typ-B-Läsionen fanden sich signifikant mehr α-Aktin- und F VIII-positive Blutgefäße als bei Patienten ohne solche Läsionen (p < 0,05). 4. Die Ergebnisse der histomorphometrischen Untersuchungen unterschieden sich nicht signifikant wenn die Patienten anhand des Schweregrades der TVP in den koronarangiographischen Untersuchungen gruppiert wurden.

Schlussfolgerungen

1. Koronarangiographische Untersuchungen sind als diagnostische Screeningmethode für die Feststellung einer TVP nach HTx geeignet. 2. Die Stanford-Klassifikation ist sensitiver für die Feststellung einer TVP mit mikrovaskulärer Beteiligung als die ausschließliche Graduierung nach Schweregrad und Lokalisation von Koronararterienstenosen (Gensini Score). 3. Minimale vaskuläre Veränderungen in der koronarangiographischen Untersuchung sind mit nur diskreten Veränderungen in der terminalen Strombahn assoziiert.

Summary

Background

Graft vessel disease (GVD) is one of the main limiting factors to long-term survival after heart transplantation (HTx). We set out to ascertain whether there is an association between changes within the terminal vascular system and angiographic findings according to the Stanford classification and the Gensini coronary scoring system.

Material and methods

We studied 30 patients (8 women, 22 men, mean age at HTx 48 years). In right ventricular rejection control samples (n = 209) harvested during the first 14 months after HTx, immunohistochemical investigations were performed with α-actin (smooth muscle cells, SMCs) and factor (F) VIII (250 000 kDa, v. Willebrand factor in endothelial cells, indicator for membrane injury). Patients’ first and last coronary angiographies (mean time after HTx 8 and 44 months) were evaluated. According to the angiographic findings graded by the Stanford classification and the Gensini coronary scoring system patients were divided into a GVD and a control group. All data were analyzed using χ2 and Mann-Whitney tests.

Results

All patients with angiographic evidence of extensive stenotic lesions showed significantly more α-actin- and F VIII-positive blood vessels (p < 0.05). We found a positive correlation of α-actin- and F VIII-positive blood vessels with lesions typified by the Stanford classification as well as the total Gensini score (p < 0.05). In patients with type B lesions there were significantly more α-actin- and F VIII-positive blood vessels than in patients without such lesions (p < 0.05). The results of histomorphometric data did not differ significantly when patients were sub-grouped according to their different degrees of GVD in coronary angiography.

Conclusions

Coronary angiography is a suitable diagnostic tool to screen for GVD after HTx. The Stanford classification seemed to be more sensitive for the detection of GVD involving the microvascular system than the exclusive description of the degree and localization of stenotic lesions in CA. Minimal vascular changes in coronary angiography are associated with only discrete changes within the terminal vascular system.
