Summary
The development of new cardiopulmonary bypass technologies, special surgical instruments, as well as intrathoracic video systems facilitates mitral surgery via a small right thoracotomy.
Since September 1996, 35 patients (17 male and 18 female) have undergone minimally invasive mitral valve surgery at our department. Mean age was 58±11 years. In 18 patients the Port-Access™ system with an endoaortic balloon clamp was used, 17 patients had video-assisted surgery with a transthoracic aortic crossclamp. Fourteen mitral valves were replaced, 21 were repaired. In 21 cases a complex mitral valve procedure was performed. Mean length of incision was 7 cm. Mean operating time lasted 265±43 min, mean bypass time 154±33 min, mean crossclamp time 94±27 min.
Despite initially longer operation and cardiopulmonary bypass time, clinical outcome and postoperative valve function were good. The crossclamp technique is simpler to perform and avoids the specific risk factors associated with the Port Access™ method. The cosmetic results are excellent and the risk of wound infection is reduced. Up to now improved recovery during hospital stay has not been realized.
Zusammenfassung
Der Einsatz neuer Technologien der extrakorporalen Zirkulation, neuem Spezialinstrumentarium, sowie die Verwendung von intrathorakalen Visualisierungssystemen ermöglicht die Durchführung von Operationen an der Mitralklappe über eine limitierte rechts-anterolaterale Thorakotomie.
Seit 1997 wurden an unserer Klinik insgesamt 35 Patienten (17 Männer, 18 Frauen) über diesen limitierten Zugang operiert. Das Durchschnittsalter betrug 58±11 Jahre. 18 Patienten wurden unter Einsatz des PortAccess™-Systems mit endoaortaler Ballonklemme und 17 Patienten mit einer videoassistierten Methode unter Verwendung einer transthorakalen Aortenklemme operiert. In 21 Fällen wurde dabei ein komplexer Mitralklappeneingriff durchgeführt. 14 Mitralklappen wurden ersetzt, 21 wurden rekonstruiert. Die durchschnittliche Inzisionslänge war 7,0±1,2 cm. Die Operationsdauer betrug 265±43 min, die Bypasszeit 154±33 min und die Aortenklemmzeit 94±27 min.
Trotz derzeit noch deutlich längerer Operationsdauer und längerer extrakorporaler Zirkulationszeit sind die klinischen Ergebnisse gut, insbesondere was die postoperative Klappenfunktion betrifft. Die Verwendung der transthorakalen Aortenklemme ist weniger aufwendig und vermeidet die spezifischen Risiken, die mit der PortAccess-Methode assoziiert sind. Das kosmetische Ergebnis ist exzellent und das Risiko von Wundheilungsstörungen reduziert. Eine raschere Erholung während des stationären Aufenthaltes wurde jedoch nicht beobachtet.
