Zusammenfassung
Hintergrund
Die Messung und Objektivierung der Haltungsregulation ist sehr aufwändig und schwierig, da es sich beim posturalen System um ein komplexes, multimodal beeinflusstes und multilokuläres System handelt. Bislang lassen sich Aussagen zu etwaigen Interventionseffekten in der Regel nur auf der Produktebene, jedoch nicht auf der Prozessebene treffen.
Fragestellung
Ziel dieser Untersuchung war es, ein neues posturografisches Messsystem, das Interaktive Balancesystem (IBS), auf seine interne Validität hinsichtlich der Parametrisierung der posturalen Subsysteme mithilfe der Fourier-Analyse zu prüfen.
Patienten und Methodik
Anhand von drei Fall-Kontroll-Studien (F 1: Sehbehinderte, F 2–4: Cochlear-Implant-Patienten, F 7–8: Kleinhirn- und Parkinson-Patienten) und einer Gruppe mit artifiziell induzierter Störung der Somatosensorik (F 5–6: Kälteapplikation) wurde die frequenzanalytische (FFT) Repräsentation der einzelnen posturalen Subsysteme (F 1: visuell, F 2–4: peripher-vestibulär, F 5–6: somatosensorisch; F 7–8: zentral, zerebellär-nigrostriatal) quasiexperimentell mit dem IBS überprüft.
Ergebnisse und Diskussion
Auf der Basis der Frequenzbereiche (0,03–3,0 Hz) der Fourier-Analyse scheint es möglich zu sein, valide und reliable Rückschlüsse bezüglich der posturalen Subsysteme zu ziehen. Veränderungen im Bereich des somatosensorischen Systems können ebenfalls an einem Frequenzbereich (F 5–6) festgemacht werden. Eine Störung im somatosensorischen System tangiert zwar die Frequenzbereiche F 1 und F 7–8 signifikant, was aufgrund der Komplexität dieses Systems auch nicht überrascht. Jedoch waren signifikante Veränderungen im Frequenzbereich F 5–6 nur bei expliziter somatosensorischer Einflussnahme zu beobachten.
Schlussfolgerung
Die Zuordnung der Frequenzbereiche der Fourier-Analyse zu den posturalen Subsystemen stellt einen Schritt zur Quantifizierung und Parametrisierung sensomotorischer Interventionseffekte dar. Sie sollte jedoch keinesfalls als endgültig betrachtet werden. Unter Umständen sind einzelne Frequenzbereiche auch als Indikatoren für mehrere Systeme in Betracht zu ziehen, was in weiteren Untersuchungen abzuklären ist.
Abstract
Introduction
Measurement and objectivisation of postural regulation is very demanding and difficult, because the postural system is a complex, multimodally influenced and multilocated system. Until now response to possible interventional effects could only be determined on a product basis, but not on a process-oriented basis.
Hypothesis
The goal of this study was to evaluate a new posturographic measurement system (interactive balance system, IBS) regarding internal validity towards the parametrisation of postural subsystems using Fourier analysis.
Patients and methods
Three case control studies (F1: visually impaired group, F2–4: cochlear implant group, F7–8: cerebellar lesions, Parkinson group) and a group with artificially induced somatosensory lesion (F5–6: cold application) were used to evaluate the frequency analytic (FFT) representation of different postural subsystems (F1: visual, F2–4: peripheral vestibular, F5–6: somatosensory, F7–8: central, cerebellar-nigrostriatal) with IBS.
Results and discussion
The Fourier analysis with a frequency range of 0.03–3.0 Hz allows a valid and reliable evaluation of postural subsystems. Changes of the somatosensory system can also be detected in a specific frequency range (F 5–6). A lesion of the somatosensory system also affects frequency parameters F1 and F7–8, which can be expected due to the complexity of the system. Significant changes however were only observed with special somatosensory triggering.
Conclusion
The correlation of frequency parameters of the Fourier analysis to postural subsystems represents a step towards quantification and parametrisation of somatosensory interventional effects. However, it is far from being totally conclusive; under certain conditions single frequency ranges may act as indicators of different systems, which has to be investigated in further studies.
