Abstract
Fatal complications of peptic ulcer disease usually involve either perforation or hemorrhage. The case of a 27-year-old female is reported who was found dead at her home address. The autopsy revealed a perforated pyloroduodenal ulcer with localized abscess formation in the lesser sac behind the stomach and established peritonitis. In addition, there was significant hemorrhage with filling of the stomach with fresh blood and the intestine with large amounts of fresh blood and melena. Death was caused by the synergistic effects of both sepsis and significant upper gastrointestinal hemorrhage due to a perforating and penetrating peptic ulcer. Rarely cases of upper gastrointestinal disease may involve more than one fatal mechanism.
Zusammenfassung
Eine Vielzahl gastrointestinaler Erkrankungen kann eine rapide klinische Verschlechterung bis hin zum Tod des Betroffenen verursachen. Gastrointestinale Erkrankungen können in allen Altersstufen auftreten und involvieren sowohl kongenitale als auch erworbene Läsionen. An letal verlaufenden Komplikationen von Magengeschwüren sind meistens Perforationen oder Hämorrhagien beteiligt. Der vorliegende Beitrag berichtet über eine 27 Jahre alt gewordene Frau mit bekannten Depressionen und Anorexia, die 3 Tage vor Todeseintritt über abdominelle Schmerzen geklagt hatte. Sie hatte eine ärztliche Abklärung verweigert und war später tot in ihrer Wohnung aufgefunden worden. Perforation und Blutung hatten sich zur selben Zeit ereignet und resultierten in einer seltenen sowie komplexen Aneinanderreihung sich wechselseitig beeinflussender Mechanismen mit letalem Ausgang. Bei der Autopsie fanden sich ein perforiertes pyloroduodenales Ulkus mit lokalisierter Abzessbildung in dem kleinen Sack hinter dem Magen und eine etablierte Peritonitis. Zusätzlich zeigten sich signifikante Hämorrhagien, Füllung des Magens mit frischem Blut sowie des Darms mit großen Mengen frischen Bluts und Teerstuhl. Todesursächlich waren die synergistischen Auswirkungen einer Sepsis und der signifikanten oberen gastrointestinalen Blutung, ausgelöst durch das perforierende und penetrierende Magengeschwür. In seltenen Fällen können obere gastrointestinale Erkrankungen mehr als einen letal wirkenden Mechanismus zur Folge haben.
