Zusammenfassung

Die Letalität durch Drogenkonsum ist einer von fünf Kernindikatoren zur Erfassung der Drogenproblematik in der Europäischen Union (EU), der von der Europäischen Beobachtungsstelle für Drogen und Drogensucht (EBDD) entwickelt worden ist. Ziel ist dabei, die Zählweisen der EU-Staaten durch ein harmonisierendes Konzept zu ersetzen. Hierzu wurden auf der International Classification of Diseases (ICD) basierende nationale Todesursachenregister und forensische/polizeiliche Spezialregister für Drogentodesfälle einbezogen. Der neu entwickelte Standard bildet Trends der direkten Drogenmortalität vergleichend ab. Defizite zeigen sich bei substanzbezogenen Analysen. Ein Problem bleibt, dass keine Vergleichsanalysen der Leichenschausysteme im Hinblick auf ihre Sensitivität für potenzielle Drogentodesfälle in der EU als Basisindikator für die Aussagekraft der Mortalitätsstatistik vorliegen. Auch der Einbezug vorhandener rechtsmedizinisch-toxikologischer Informationen in die ICD-basierten Todesursachenregister mit nationaler Reichweite sollte optimiert werden. Eine Änderung der ICD-10-Kodierungsregeln für Drogentodesfälle kann den Informationsverlust reduzieren. In Entwicklung befindet sich ein Konzept zur Erfassung auch indirekter Todesursachen, um ein vergleichendes Bild der Gesamtmortalität durch Drogenkonsum zeichnen zu können.

Abstract

Drug-related mortality is one out of five key indicators for the registration of drug problems in the European Union which has been developed by the European Monitoring Centre for Drugs and Drug Abuse (EMCDDA). This new concept is a consensus model using the International Classification of Diseases (ICD) which forms the basis of registration procedures and datasets and allows a direct comparison of global trends in drug mortality among EU member states. However, substance-related analyses are still not adequate. A general problem is the lack of specific information on the quality of drug-related death evaluation which depends on the sensitivity of national post-mortem routines. There is insufficient information about the lack of forensic and toxicological information when processed for registration in national ICD-based mortality registries. An amendment of coding rules for drug-related deaths according to ICD-10 has been proposed which may reduce the loss of information through coding practices and makes ICD-based statistics more specific. A new indicator which could compare the total burden of drug-related mortality among EU member states is being developed.
