Zusammenfassung
Posttraumatische Slope-Veränderungen spiegeln sich in der aktuellen Literatur kaum wider, haben aber für die Kinematik des Kniegelenks durchaus eine erhebliche Relevanz. Unterschieden werden kann zwischen intraartikulären und extraartikulären Deformitäten. Darüber hinaus können die iatrogenen postinterventionellen Slope-Veränderungen abgegrenzt werden. Die Therapie der Wahl muss individuell geplant werden. Die intraartikulären Deformitäten treten oft nach Tibiakopffrakturen auf, welche meist lateral angesiedelt sind. Therapie der Wahl ist in diesem Fall eine isolierte laterale Hemianhebeplastik. Die extraartikulären Deformitäten müssen meist im CORA („center of rotation and angulation“) korrigiert werden. Hier muss jede Dimension der Deformität mit analysiert und ggf. eine Korrektur durchgeführt werden. Die iatrogenen Deformitäten, meist nach aufklappender Tibiakopfosteotomie, entstehen sehr häufig durch eine inkomplette Durchtrennung der dorsalen Kortikalis und damit einem zu weiten ventralen Aufklappen des Osteotomiespalts. In diesem Fall muss in der Korrekturosteotomie die frontale und die sagittale Achse korrigiert werden.
Abstract
Posttraumatic slope deviations are rarely dealt with in currently available publications but have a substantial relevance for the kinematics of the knee. They can be divided into intra-articular and extra-articular deformities. Additionally, iatrogenic slope deformities need to be seen as a third subdivision of these deformities. In each case individual planning of the correction is essential. Intra-articular deformities often occur after tibial head fractures and are mostly located in the lateral aspect. In this case the treatment of choice is isolated lateral unicondylar osteotomy. Extra-articular deformities mostly need to be corrected by center of rotation and angulation (CORA) and every dimension of the deformity needs to be analyzed and addressed afterwards. Iatrogenic deformities mostly appear after medial open wedge high tibial osteotomy very often due to incomplete severance of the dorsal corticalis. This leads to an excessive ventral enlargement of the osteotomy gap resulting in a slope deviation. Corrective osteotomy in these cases needs to address both the frontal and sagittal planes.
