Zusammenfassung
Hintergrund
Die exakte Wiederherstellung des Gelenkplateaus ist operatives Ziel bei der Frakturversorgung von Tibiakopffrakturen. Für ausgewählte Fälle steht dabei mittlerweile die Anwendung der Ballontibioplastie zur Verfügung. Der vorgestellte Kurzbeitrag und das beigefügte Videomaterial stellen die minimal-invasive Technik der Tibioplastie anhand eines konkreten Versorgungsbeispiels anschaulich dar.
Methode
Die ballongeführte Technik bietet durch einen dosierten Druckaufbau unterhalb der Impression die Möglichkeit einer kontrollierten Reposition. Die relativ große Ballonoberfläche sorgt dafür, dass gleichzeitig mehr Knochen angehoben werden kann. Die Positionierung des Tibioplastieballons erfordert operative Präzision. Die Ballonaufrichtung schafft einen gut definierten Knochenhohlraum mit bekanntem Volumen. Die Frakturreposition wird durch Augmentation mit Calciumphosphatzement gesichert. Mögliche Risiken sind Zementaustritt und sekundärer Repositionsverlust.
Ergebnisse
Die bisherigen Ergebnisse sind hoffnungsvoll, vergleichende Langzeitergebnisse oder größere Fallserien sind in der orthopädischen Literatur bisher jedoch nicht beschrieben. Deshalb sollte die Indikationsstellung zur Ballontibioplastie bei Tibiakopfimpressionsfrakturen differenziert erfolgen.
Abstract
Background
The aim of surgical treatment of fractures of the tibial head is an exact reconstruction of the joint plateau. For this purpose the method of balloon tibioplasty is now available in selected cases. This article and the accompanying video material illustrate the minimally invasive technique of tibioplasty using an actual example of patient treatment.
Methods
This technique offers gentle reduction by slow expansion of the balloon. The large balloon surface ensures that more bone can be lifted carefully at once in order to achieve the anatomical position. The positioning of the balloon requires surgical precision. Balloon reduction creates a well-defined bone cavity of known volume and is stabilized using calcium phosphate cement. Possible risks are cement leakage and secondary loss of reduction.
Results
Thus far, results are promising, but long-term results are still lacking. Therefore, the indication should be made carefully and differentiated.
