Zusammenfassung
Eine 18-jährige Erstgravida wird mit 40+2 SSW bei Gestationsdiabetes und bekannter Thrombophilie zur Geburtseinleitung aufgenommen. Die Entbindung erfolgt am selben Tag per Notsectio bei fetaler Bradykardie. Post partum kommt es zu generalisierten, tonisch-klonischen Krampfanfällen. Eine Sinusvenenthrombose und eine intrakranielle Blutung können per Bildgebung ausgeschlossen werden; die Diagnose lautet PRES (posteriores reversibles Enzephalopathiesyndrom). Bei völligem Fehlen übriger Gestosesymptome liegt eine Eklampsie mit fulminantem, postpartalem Verlauf vor, deren bildmorphologisches Korrelat die posteriore reversible Leukenzephalopathie ist.
Abstract
An 18-year-old primigravida was admitted to hospital at 41 weeks of gestation. She had gestational diabetes and hereditary thrombophilia. Delivery of the baby was by emergency C-section because of fetal bradycardia. The patient experienced postpartum spontaneous, general seizures. Computed tomography imaging showed no sign of intracerebral hemorrhage or sinus thrombosis. Posterior reversible leukoencephalopathy syndrome was diagnosed, which is often seen in patients with preeclampsia or eclampsia. Our patient, however, showed no other symptoms of preeclampsia or gestosis.
