Zusammenfassung

Die Zystoskopie und die transurethrale Resektion sind in Kombination mit der urinzytologischen Diagnostik in der europäischen Leitlinie festgeschriebener Bestandteil in der Behandlung des Urothelkarzinoms der Harnblase. Bei Low-grade-Urothelkarzinomen mit bekannter hoher Rezidiv-, aber äußerst geringer Progressionsneigung stellt sich vom Grundsatz die Frage, ob eine Zusatzdiagnostik erforderlich ist. Bei High-grade-Tumoren kommt der frühen Diagnosestellung wegen der hohen Progressionsneigung besondere Bedeutung zu. Die fluoreszenzgestützte Diagnostik und das „narrow band imaging“ sind optische Verfahren, die zur Steigerung der Detektionsrate v. a. des Carcinoma in situ eingesetzt werden können. Die vorliegende Arbeit gibt einen Überblick über den aktuellen Stellenwert beider Verfahren.

Abstract

Cystoscopy and transurethral resection in combination with urinary cytology are integral parts of the diagnosis and therapy of urothelial cell carcinoma of the bladder. Given the fact that low grade bladder cancer has high recurrence and extremely low progression rates, the benefit for additional diagnostics needs to be further evaluated. In high grade tumors, early and accurate diagnosis is of high importance. Fluorescence cystoscopy and narrow band imaging are procedures to increase the detection rate. This article provides an overview of the current value of these two procedures.
