Zusammenfassung
Epidemiologische Daten zu chronischer Prostatitis/chronischem Beckenschmerzsyndrom (CP/CPSS) belegen die große Bedeutung des Prostatitissyndroms in der urologischen Sprechstunde. Bis auf die seltenen Fälle einer bakteriellen Prostatitis sind die zahlreichen somatischen Erklärungsmodelle der chronischen Prostatitis/des chronischen Beckenschmerzsyndroms (CP/CPSS) nur unvollständig in der Lage, die pathogenetischen Zusammenhänge zu erklären; therapeutische Ansätze sind oft in ihrem Erfolg unbefriedigend. Psychosomatische Faktoren werden demgegenüber nur rudimentär gewürdigt. Bestimmte Persönlichkeitsmerkmale, insbesondere Somatisierung und Depression, aber auch Ängstlichkeit, Hypochondrie und abgeschwächte männliche Identität scheinen nach testpsychologischen Ergebnissen eine wesentliche Rolle zu spielen, bedürfen aber noch weiterer Evaluierung. Diagnostische und therapeutische Ansätze sollten in Zukunft den komplexen Verflechtungen somatischer und psychogener Faktoren des CP/CPPS Rechnung tragen.
Abstract
Epidemiologic data have demonstrated the significance of chronic prostatitis/chronic pelvic pain syndrome (CP/CPSS) for the urological practice. Apart from the rare cases of bacterial prostatitis numerous somatic models have failed to completely explain pathogenetic interrelations; therapeutic attempts frequently have produced unsatisfactory results. Additionally, psychosomatic factors have often been neglected. A number of personality variables have turned out to play an important role in questionnaire-based studies, namely somatization and depression, as well as anxiety, hypochondriasis, and weak masculine identity. However, these preliminary results need further evaluation. In the future, diagnostic and therapeutic strategies should pay attention to the complex interweavement of somatic and psychogenic factors in the pathogenesis of CP/CPSS.
