Hemiparese nach stumpfem enoralem Trauma bei einem 1jährigen Mädchen

Zusammenfassung

Ein 1jähriges Mädchen zog sich bei einem Sturz mit einem Bleistift im Mund einen posttraumatischen Verschluß der A. carotis interna zu. Der daraus resultierende ausgedehnte Infarkt, das massive Hirnödem und ein passager auftretendes SIADH führten etwa 18 h später zum lebensbedrohlichen zerebralen Koma. Unter Hirnödemtherapie und i.v. Heparinisierung stabilisierte sich das neurologische Zustandsbild nach 1 Woche. Innerhalb 1 Jahrs lernte das Kind wieder zu gehen. Eine mäßiggradige Hemiparese links blieb jedoch als Restzustand trotz intensiver Physiko- und Ergotherapie bestehen.

Diskussion: Eine gezielte Traumaanamnese bei ungeklärter erworbener Hemiparese des Kindesalters ist wegen des luziden Intervalls von entscheidender Bedeutung. Frühzeitige Heparisierung wird als Therapie der Wahl empfohlen, um ein intraluminales Wachstum des Thrombus zu verhindern.

Summary

A one year old girl suffered a traumatic occlusion of the internal carotid artery after a fall with a pencil in her mouth. The resulting stroke with massive cytotoxic edema and an intermittent SIADH caused a life threatening cerebral coma 18 hours later. Due to therapy of the cerebral edema and anticoagulation with intravenous heparin the neurologic condition stabilized after one week. Within one year the child regained the ability to walk. A left sided hemiparesis persisted despite intensive physical- and occupational therapy.

Discussion: A precise history of trauma in obscure stroke cases during childhood is very important, because the onset of symptoms may be delayed for hours or days following the accident. Early anticoagulation is the treatment of choice to avoid the intraluminal propagation of the thrombus.
