Zusammenfassung
Hintergrund
Anorektale Fisteln und Abszesse werden im Kindesalter häufig wie Abszesse und Fisteln des Erwachsenen behandelt. Ziel dieser Studie war es zu klären, ob es sich im Säuglings- und Kleinkindalter ebenfalls um eine kryptoglanduläre Infektion handelt oder ob trotz gleicher Symptome ein grundlegend unterschiedliches Krankheitsbild vorliegt.
Material und Methoden
Im Rahmen einer retrospektiven Studie wurden alle Kinder und Jugendliche im Alter von 0 bis 16 Jahren, die aufgrund eines primären anorektalen Abszesses oder einer primären Analfistel behandelt worden waren, hinsichtlich Alter, Geschlecht, Diagnose und Lokalisation analysiert.
Ergebnisse
Im Zeitraum von 20 Jahren wurden 183 Kinder (♀ = 22, ♂ = 161) mit der klinischen Diagnose eines anorektalen Abszesses oder einer Analfistel behandelt. Allein 70,5 % der Kinder wurden innerhalb des 1. Lebensjahres und weitere 9,3 % innerhalb des 2.Lebensjahres behandelt (= 79,8 %; Gruppe 1; n = 146). Nur 20,2 % waren bei der Behandlung 3 Jahre und älter (Gruppe 2; n = 37). In der Gruppe 1 überwog das männliche Geschlecht signifikant (11:1), während dies in der Gruppe 2 nahezu dem des Erwachsenenalters entsprach (2,6:1). Auffällig war, dass sich in der Gruppe 1 fast zwei Drittel aller Fisteln/Abszesse auf einer Horizontalen zwischen 3:00 und 9:00 Uhr Steinschnittlage fanden.
Schlussfolgerung
Sowohl die divergierende Alters- und Geschlechtsverteilung als auch die Auffälligkeiten hinsichtlich der Lokalisation lassen die Vermutung einer kongenitalen Ätiologie von Fisteln im Säuglings- und Kleinkindalter zu.
Abstract
Background
Perianal abscesses and anal fistulas in childhood are commonly treated like abscesses and fistulas in adults. The aim of this study was clarify whether they are cryptoglandular infections as in adults and whether they are two different kinds of underlying diseases, just with the same symptoms.
Material and methods
We retrospectively analyzed all children and adolescents between 0 and 16 years of age who underwent surgical treatment for a primary perianal abscess or a primary anal fistula. The records were analyzed with respect to age at appearance of lesions, sex, diagnosis and anatomical localization of lesions.
Results
Within a time period of 20 years a total of 183 children (♀ = 22, ♂ = 161) with the clinical diagnosis of anorectal abscesses or anal fistulas were treated. Of the children 70.5% were treated during their first year of life and another 9.3% up to the age of 2 years (group 1 79.8%, n = 146). Only 20.2% were 3 years and older (group 2, n = 37). Group 1 contained significantly more male infants (m:f = 11:1); however, an almost equal sex distribution was detected for group 2 (m:f = 2.6:1), a similar result to adults. It was conspicuous that in group 1 nearly two thirds of the anal fistulas and abscesses were localized horizontally between 3:00 and 9:00 h in a lithotomy position.
Conclusion
Divergences in age and sex, as well as in anatomical localization suggest a congenital etiology for anal fistulas and perianal abscesses in infants and children.
