Zusammenfassung
Vorgestellt wird der Fall einer 8½-jährigen Patientin, die mit körperlicher Mattigkeit, Erbrechen und Vigilanzminderung stationär zur Abklärung eingewiesen wurde. Das Kind hatte ein halbes Jahr vorher ein reanimationspflichtiges Schockgeschehen erlitten, dessen Ursache nicht geklärt werden konnte. Eine Hypothyreose wurde seinerzeit im Rahmen der Abklärung diagnostiziert und mit Schilddrüsenhormonen behandelt. Die laborchemischen Untersuchungen bei Aufnahme des Kindes zeigten eine Hyponatriämie, einen niedrig normalen Blutzucker, ein deutlich erhöhtes TSH unter Substitutionstherapie bei normalen peripheren Hormonen und eine kompensierte metabolische Azidose. Klinisch fanden sich eine Braunverfärbung der Haut und ein erniedrigter Blutdruck sowie eine stagnierende Gewichtsentwicklung nach massivem Gewichtsverlust von 8 kg im Rahmen des stationären Aufenthaltes 6 Monate vorher in einer auswärtigen Klinik. In diesem Kontext stellten wir die Verdachtsdiagnose eines Cortisolmangels und vermuteten einen Zusammenhang mit der bereits gestellten Diagnose der Schilddrüsenunterfunktion. Die diagnostischen Schritte zur definitiven Diagnose eines APS Typ 2 (Schmidt-Syndrom) und die therapeutischen Maßnahmen werden dargelegt und diskutiert.
Abstract
This article presents the case of an 8 ½ year old girl who was admitted to hospital after exhibiting symptoms of weakness, vomiting and decreased levels of vigilance. The girl had suffered from shock of unknown origin that had required resuscitation 6 months earlier. At the time she had been diagnosed with hypothyroidism and was treated with thyroid hormones. Her first blood tests in our hospital indicated hyponatraemia, low blood sugar, compensated metabolic acidosis and considerably high TSH with normal peripheral hormones. Clinical findings were brown skin discoloration, low blood pressure, and weight stagnation after tremendous weight loss, which occurred during her first hospitalisation (6 months earlier). In this context, we made a tentative diagnosis of cortisol deficiency (Addison’s disease) and a link with hypothyroidism was also suspected. In this paper, we demonstrate the diagnostic procedures for Schmidt’s syndrome (APS type 2) and its therapeutic measures.
