Zusammenfassung
Hintergrund
Bei Kardiomyopathien und Myokarditiden im Kindes- und Jugendalter kommt differentialdiagnostisch eine unerwünschte Arzneimittelwirkung (UAW) in Betracht.
Ziel der Arbeit
Ausgehend von einem spontan gemeldeten Verdachtsfall unter Therapie mit Clozapin wurde untersucht, zu welchen Arzneimitteln diese UAW gemeldet wurde.
Material und Methoden
Material und Methoden: In der Datenbank des deutschen Spontanmeldesystems wurden alle Verdachtsmeldungen einer Kardiomyopathie oder Myokarditis unter Kindern und Jugendlichen selektiert und mittels der Case/Non-Case-Methode die Reporting Odds Ratios (RORs) der UAW berechnet.
Ergebnisse
In 39 von knapp 14.000 UAW-Verdachtsfällen bei Kindern und Jugendlichen wurde zu einem Arzneimittel eine Kardiomyopathie oder Myokarditis gemeldet. Eine Assoziation wurde neben dem am häufigsten verdächtigten Clozapin (ROR: 29,2; 95 % CI: 13,6–62,6, p < 0,001) für Mesalazin (ROR: 32,2; 95 % CI: 9,5–109,4; p < 0,001) gefunden.
Diskussion
Unter Clozapin und Mesalazin sollte bei Auftreten unspezifischer Symptome an eine arzneimittelbedingte Kardiomyopathie oder Myokarditis gedacht werden. Diese UAW werden im Kindes- und Jugendalter seltener gemeldet als erwartet. Da es sich bei der Kardiomyopathie um einen Oberbegriff handelt, könnte die Nomenklatur zur geringen Zahl spezifischer Meldungen beigetragen haben. Ein weiterer Grund könnte sein, dass diese UAW zumeist in der pädiatrischen Onkologie auftreten, durch Therapieoptimierungsstudien erfasst werden und nicht in das Spontanmeldesystem eingehen.
Abstract
Background
If cardiomyopathies and myocarditides in children and adolescents occur, an adverse drug reaction (ADR) should be considered.
Objectives
Starting from a spontaneously reported case related to clozapin, a database search was conducted to identify drugs reported with these ADR.
Material and methods
The German spontaneous reporting database was searched to identify all spontaneous reports with a cardiomyopathy or myocarditis in children and adolescents. With the case/non-case method ADR reporting odds ratios (RORs) were calculated.
Results
In 39 of nearly 14,000 suspected ADR-cases in children and adolescents a cardiomyopathy or myocarditis was reported. Besides clozapine, that has been suspected most often (ROR: 29.2; 95 % CI: 13.6–62.6, p < 0.001) an association was found for mesalazine (ROR: 32.2; 95 % CI: 9.5–109.4; p < 0.001).
Conclusion
If non-specific symptoms occur with clozapin or mesalazine treatment, one should consider the possibility of a drug-induced cardiomyopathy or myocarditis. These ADRs were reported less frequently in children and adolescents than we expected. As cardiomyopathy is a generic term, the nomenclature may have contributed to the small number of specific reports. The results may also reflect the fact that the investigated ADRs most often occur in paediatric oncology, are being covered by therapy-optimization trials, and were not reported to the German spontaneous reporting database.
