Komplikationen von Kunsthaarimplantationen

Zusammenfassung.

Fünf männliche Patienten, Durchschnittsalter 34 Jahre, die an den Folgen einer Kunsthaarimplantation litten, wurden zwecks Erfüllung gerichtlicher Gutachtenaufträge untersucht. Pro Sitzung waren 1000 Kunsthaare aus Polyätheramid implantiert worden, bei einem Patienten mehrfach. In allen Fällen war eine modifizierte Implantationstechnik mit feiner Nadel und subkutaner Verknotung in insgesamt 3 Instituten durchgeführt worden. 3 Patienten zeigten Frühkomplikationen nach 4 – 8 Wochen mit bakterieller Follikulitis, bei 2 Patienten trat diese erst nach 3 – 6 Monaten auf. Als mögliche Auslöser wurden bei 2 Patienten das Tragen eines Motorradhelmes bzw. der Aufenthalt in tropischem Klima diskutiert. Bei einem weiteren Patienten trat nach einem Saunabesuch eine kosmetisch störende Kräuselung des Kunsthaares auf. Das implantierte Haar war bei allen Patienten weitgehend ausgefallen (100% bei 2 Patienten nach 9 – 12 Monaten, 50 – 75% bei 3 Patienten nach 7 Monaten bis 2 Jahren). Alle Patienten wiesen kosmetisch störende feine Narben und Pigmentverschiebungen auf. Trotz offenbar reduzierter Komplikationsrate bleibt auch die neue Implantationstechnik mit modernen Kunsthaarmaterialien medizinisch bedenklich und kann wegen der z. T. irreversiblen Folgezustände nicht empfohlen werden.

Abstract.

Five men (average age 35 years) suffering from the sequelae of hair implants were examined in the course of claims for legal compensation. Polyether amide hair fibres had been implanted, 1000 per patient and session. In all cases the improved implantation technique with a needle and subcutaneous knotting had been used in a total of three institutions. Three patients developed bacterial folliculitis after 4 – 8 weeks; in the other two patients this developed later, after 3 – 6 months. In two patients the possible triggering event was the wearing of a motorcycle helmet and a vacation in a tropical climate respectively. In another patient the artificial hair curled considerably after he visited a sauna. The implanted hair had fallen out almost completely in all cases (100% in two patients after 9 – 12 months, 50 – 75% in three patients after 7 months to 2 years). All patients showed cosmetically disturbing small scars and pigmentary changes. Despite an apparently improved complication rate, the new technique of hair fibre implantation remains a doubtful procedure and cannot be recommended in view of possible permanent sequelae.
