Zusammenfassung
Chronischer Pruritus ist eine schwer therapierbare Begleiterscheinung zahlreicher Dermatosen und innerer Erkrankungen. Häufig sind ursächliche und konventionelle Therapien unwirksam und prolongieren den Pruritus. Dadurch entstehen dem Gesundheitssystem nicht unerhebliche Kosten, die durch ein rationelles Therapiekonzept deutlich gesenkt werden können. Zu der topischen und systemischen, symptomatisch wirksamen Therapie wurden in den letzten Jahren umfangreiche neue Erkenntnisse gewonnen. Aktuell stehen neue Therapien zur Verfügung, die in unterschiedliche, pruritogene Mechanismen eingreifen. Eingesetzt werden Substanzen, die die kutane Pruritusentstehung blockieren (z. B. Antihistaminika, Leukotrienrezeptorantagonisten, Cannabinoidagonisten) bzw. die zentrale Effekte mediieren wie die Hemmung der Weiterleitung von Pruritus auf Rückenmarksebene (z. B. Naltrexon, Gabapentin) oder Unterdrückung der zerebralen Juckempfindung (z. B. Antidepressiva, Neuroleptika). Zumeist ist eine Kombination verschiedener Therapieprinzipien notwendig, um chronischen Pruritus nachhaltig zu unterdrücken. Um hier ökonomisch vorzugehen, empfiehlt es sich, schrittweise unter Einhaltung der Möglichkeit der kassenärztlichen Rezeptierbarkeit aufzudosieren.
Abstract
Chronic pruritus is a difficult-to-treat problem which accompanies many dermatologic and systemic diseases. Frequently, therapy is ineffective and prolongs the pruritus. Thus significant costs arise for the health system, which are clearly lowered by a rational therapy concept. Extensive knowledge about both topical and systemic therapy of pruritus has been acquired in recent years. Currently, new therapeutic approaches are available, which intervene in different pruritogenic mechanisms. Substances can block the cutaneous pruritus induction (e.g. antihistamines, leukotriene antagonists, cannabinoid agonists) or mediate central effects such as the inhibition of pruritus transmission via the spinal cord (e.g. naltrexone, gabapentin) or the suppression of the cerebral itch perception (e.g. antidepressants, neuroleptics). Usually combining several therapeutic principles is necessary in order to suppress chronic pruritus with lasting effect. The most effective approach is to gradually combine therapies, always considering both the medical and financial impacts.
