Twiddler-Syndrom bei einem links pektoral implantierten Defibrillator

Zusammenfassung

Das Twiddler-Syndrom beschreibt eine seltene Komplikation nach einer Schrittmacher-/Defibrillatorimplantation und ist durch die spontane, mehrfache Rotation des Generators in der subpektoralen Tasche gekennzeichnet. Es wird der Fall eines 78-jährigen Mannes geschildert, bei dem 7 Monate nach Defibrillatorimplantation, nach zunächst unauffälligem Verlauf, ein Exitblock festgestellt wurde. Als Ursache konnte eine 15fache Rotation des Defibrillatorgenerators um die eigene Achse verifiziert werden, die die Dislokation der Defibrillatorelektrode zur Folge hatte. Eine sichere Fixation der Elektrode und des Generators in der subpektoralen Tasche ist von höchster Wichtigkeit, um diese seltene Komplikation, insbesondere bei Patienten mit einem lockeren subkutanen Fettgewebe, zu vermeiden.

Abstract

Twiddler’s syndrome describes a rare complication following implantation of a pacemaker/defibrillator and is characterized by spontaneous, repeated rotation of the generator in the subpectoral pocket. We describe the case of a 78-year-old man who presented with an exit block 7 months after defibrillator implantation. As cause, a 15-fold rotation of the defibrillator‘s generator around its own axis could be verified, which entailed dislocation of the defibrillator’s electrode. Safe fixation of the electrode and generator in the subpectoral pocket is of paramount importance, particularly in patients with loose subcutaneous fatty tissue, to prevent this rare complication.
