Zusammenfassung
Fallbeschreibung: Wir berichten über ein 10-jähriges Mädchen mit komplexer zerebraler Fehlbildung bei Zustand nach frühkindlicher Meningoenzephalitis und shuntversorgtem Hydrozephalus, das nach einem Bagatelltrauma einen Pneumencephalus erlitt. Es bestand keine Rhinoliquorrhö; eine extra-/intrakranielle Kommunikation musste aber wegen der intraventrikulären Luft vorliegen. Eine Dysfunktion des Stuntsystems wurde klinisch und radiologisch ausgeschlossen. Weder natives MRT noch hochauflösende CT konnten zweifelsfrei den Fistelpunkt identifizieren, der im Ethmoidalzellsystem oder an der Stirnhöhlenhinterwand vermutet wurde. Für die notwendige neurochirurgische Operation war es erforderlich, den Zugangsweg sicher zu definieren. Ergänzend wurde daher eine MRT-Untersuchung mit intrathekaler Gabe von Gadolinium durchgeführt. Mittels intrathekaler Gd-Myelozisternographie konnte ein frontaler Fistelpunkt identifiziert werden. Nach erfolgreicher plastischer Deckung der linken Stirnhöhle und der linken Frontobasis sistierte der Pneumencephalus und die intraventrikuläre Luft wurde im Verlauf resorbiert. Die klinische Symptomatik bildete sich zurück.
Abstract
Case Report: We report a case of a 10-year-old girl with a history of a complex cerebral dysplasia with hydrocephalus after meningo-encephalitis during the 1st year of life. The girl developed a pneumencephalus after a mild trauma (fall on the bottom), without severe head or brain injury. Clinically evident were headache, nausea and vomiting. In this case no cerebrospinal fluid leakage was obvious, but a CSF-fistula with extra-intradural communication was recommended. Either CT nor native MRI could find the site of leakage, wich was estimated into the frontal sinus or into the ethmoid cells. Gadolinium-cisternography demonstrated exactly the site of fistula into the frontal sinus. The fistula was surgically detected and closed with dura and periost successfully. The patient tolerated the examination without complications or any indication of side effects.
