Therapeutic inhibition of platelets in acute coronary syndrome and in coronary intervention: mechanisms and clinical results

Summary

Platelets play a crucial role in the pathogenesis of atherosclerosis and especially in the final ischemic consequences such as acute coronary syndromes. Furthermore, platelets are central mediators of acute or subacute complications of coronary interventions. Therefore, therapeutic inhibition of platelet function is of major interest in cardiology. The following review describes three different therapeutic strategies for platelet inhibition and provides a representative overview on the clinical results of studies based on these strategies.

First, the mechanism of acetylsalicylicacid is described and the strong metaanalytic data demonstrating a convincing positive clinical effect is discussed. Second, the mode of action of the thienopyridines is described and initial clinical results are discussed. Third, the inhibition of the platelet integrin receptor GP IIb/IIIa is described as a potent way to block the final common pathway of platelet stimulation. The structural description of GP IIb/IIIa is followed by a structural classification of the available GP IIb/IIIa inhibitors. Clinical studies, meanwhile including several thousands of patients, are discussed based on representative examples. Finally, unresolved issues regarding the various GP IIb/IIIa inhibitors, such as differences in receptor affinity and specificity, intrinsic activation and GP IIb/IIIa inhibitor induced thrombocytopenia are, discussed.

Zusammenfassung

Thrombozyten spielen in der Pathogenese der Arteriosklerose und insbesondere in der oft resultierenden Organischämie, wie beispielsweise dem akuten Koronarsyndrom, eine entscheidende Rolle. Zudem ist die Thrombozytenaktivierung oft die Ursache für akute und subakute Komplikationen bei Koronarinterventionen. Die Möglichkeit, Thrombozyten therapeutisch zu hemmen, ist deshalb zentralem Interesse in der Kardiologie. In dieser Übersichtsarbeit werden drei unterschiedliche therapeutische Strategien zur Thrombozytenhemmung dargestellt. Zudem wird versucht, einen representativen Überblick über klinische Studien dieser drei Therapieprinzipien zu geben.

Zunächst werden der Wirkmechanismus der Acetylsalicylsäure (ASS) und die ausgesprochen positiven metaanalytischen Daten über den klinischen Einsatz von ASS beschrieben. Danach werden der Wirkmechanismus der Thienopyridine und die ersten klinischen Ergebnisse mit diese Substanzgrupe diskutiert. Als dritte therapeutische Strategie der Thrombozytenhemmung wird die Hemmung des Thrombozytenintegrins GP IIb/IIIa besprochen. Die Aktivierung dises Oberflächenrezeptors stellt die gemeinsame Endstrecke der Thrombozytenaktivierung dar. Die Struktur des GP-IIb/IIIa-Rezeptors wird dargestellt. Weiterhin wird eine Klassifikation der verfügbaren GP-IIb/IIIa-Blocker vorgenommen. Anhand von repräsentativen Beispielen werden die bisherigen klinischen Ergebnisse der unterschiedlichen GP-IIb/IIIa-Blocker besprochen. Schließlich werden offene Fragen der Therapie mit GP-IIb/IIIa-Blocker wie die Überlebensrate der Rezeptoraffinität, der Rezeptorspezifität, der intrinsich aktivierenden Eigenschaft von GP-IIb/IIIa-Blockern und möglicher Ursachen der GP-IIb/IIIa-Blocker induzierten Thrombozytopenie diskutiert.
